Marion Co. No. 573 Application for Allowance For Left Leg Applicant J. H. Stevens County Marion Amount 25 Date of Warrant Jany 16/88 Entered on Record Jany 16, 1888 W.H.H. STATE OF GEORGIA} Marion County } Personally appears J. H. Stevens of Marion County, State of Georgia, who being duly sworn on oath that he is a bona fide citizen and resident of said State, and has been such since the 7 day of June 1831; that he enlisted in the military service of the Confederate States (or of the State of _________) during the war between the States and served as a Sergt. & Lt. in Company G, of 5th Regiment of Ga Volunteers Jackson’s Brigade; that whilst engaged in such military service, on retreat of Gen'l. Bragg from the State of Ky, on the ____ day of Jan or Feby 1863, he was wounded as follows (or whilst in said service in the year 1862, he contracted disease as follows) (State full nature of wound or character of disease which causes disability) Contracted Rheumatism in the Army causing softening of the bone in left leg and leg broke in retreat from Ky by Gen’l Bragg in Jan. or Feby., 1863 near the hip. Fracture never united leg is very weak, unable to bear weight & stick has to be used to enable applicant to walk. Which wound (or disease) permanently disables and renders him practically incompetent to perform manual labor, and his arm, or leg, or ________substantially useless. Deponent desires to participate in the benefits of the Act, approved October 24, 1887, and makes application for the allowance to which he is entitled thereunder. Lowe ordy COMMISSIONED OFFICERS AFFIDAVIT STATE OF GEORGIA} Schley County Personally came before me Robert Burton of the county of Schley State of Georgia, who being duly sworn, says that he was a commissioned officer in Company G, of 5 Regiment of Ga Volunteers, and that deponent knows J. S. Stevens, and that he received the wounds (contracted the disease) in the military service as stated in his foregoing affidavit, and the wounds or disease permanently disables the said J. H. Stevens, as stated by him in said affidavit. Deponent further states that said J. H. Stevens is a bona fide citizen of this State, and resides in Marion county. Sworn and subscribed to me this ________day of ___________188 T. J. Dozier ordy Robert Burton Capt. Co. G 5th Ga. Reg. Additional Comments: J. H. Stevens received a pension in the years 1888, 1889, 1891, 1893, 1895 and 1897. In some of the other pension application he states his injury occurred during the retreat through Ky and Tenn.
