Allen of said State and County, desiring to avail himself of the Pension Act (Section 1254, Code), hereby submits his proofs, and after being duly sworn true answers to make to the following questions, deposes and answers as follows: 1. What is your name and where do you reside? Thomas J. Allen P.O. LaCrosse, Schley Co. 2. How long and since when have you been a resident of this State? resided in Schley County 64 years 3. When and where were you born? Born in 1837 4. When and where and in what company and regiment did you enlist or serve? enlisted in Schley Co., Regiment 5th Ga Company "G" May 11th 1861 5. How long did you remain in such company and regiment? four years 6. When and where was your company and regiment surrendered and discharged? 1865 surrendered and discharged near Raleigh,North Carolina 7. Were you present with your company and regiment when it surrendered? Yes 8. If not present, state specifically and clearly where you were, when you left your command, for what cause and by whose authority? I was present 9. How much can you earn per annum by your own exertions of labor? $20.00 10. What has been your occupation since 1865? FARMING 11. Upon which of the following grounds do you base your application for pension, viz: first, "age and poverty," second, "infirmity and poverty," or third, "blindness and poverty"? infirmity and poverty 12. If upon the first ground, state how long you have been in such condition that you could not earn your support? If upon the second, give a full and complete history of the infirmity and its extent? If upon the third, state whether you are totally blind and when and where you lost your sight? Having trouble for 20 years 13. What property, real or personal, or income, do you possess, and its gross value? I have one, one horse wagon woth $10.00 14. What property, real or personal, did you possess in 1894, 1895, 1896, 1898, and 1899, and what disposition, if any, by sale or gift, have you made of same? I did not have any. 15. In what county did you reside during those years, and what property did you then return for taxation? resided in Schley Co., returned no property 16. How were you supported during the years 1898 and 1899? my wife supported in 1898 & 1899. 17. How much did your support cost for each of those years, and what portion did you contribute thereto by your own labor or income? support cost about 100.00 per year & contributed $20.00 each year 18. What was your employment during 1898 and 1899? What pay did you receive in each year? My wife run a farm and I would help her when I was able to do so. 19. Have you a family? If so, who composes such family? Give their means of support? Have they a homestead? wife and one daughter--Lucinday Allen & Eula Elector(?) Allen. They have no homesttaed, ent land to support themselves & applicant 20. Are yuou recieving any pension? If so, what amount and for what disability? No. 21. Have you ever made an application for pension before? None 22. How many applications have you ever made and under what class? applied once under the indigent(?) T.J. Wimbish, of said State and County, having been presented as a witness in support of the application of T.J. Allen for pension under section 1254, Code, and after being duly sworn true answers to make to the following questions, deposes and answers as follows: 1. What is your name and where do you reside? M.W. Wimbish, Schley County, Ga. 2. Are you acquainted with T.J. Allen, the applicant; if so how long have you known him? known him for 40 years. 3. Where does he reside, and how long and since when has he been a resident of this state? Schley Co., ever since I have known him. 4. When, where and in what company and regiment did he enlist, and how do you know? Enlisted Co. G, 5th Ga. Reg. at Ellaville, Ga., on May 11, 1861. 5. Were you a member of the same company and regiment? I was 6. How long did he perform regular military duty? 4 years 7. When and where was his command surrendered? surrendered at near Raleigh, N.C. on 26 day April 1865 8. Were you present when it surrendered? I was 9. Was applicant present? He was 10. If he was not present, where was he? He was present. When did he leave his command? at surrender. For what cause? surrender. By what authority he left? by parole. How do you know all of this? Because I was present. 11. What property, effects or income has the applicant? (give means of knowledge) None, because I live near him. 12. What property, effects or income did the applicant possess in 1896, 1897, 1898, and 1899, and what disposition, if any, did he make of same? Had no property. 13. Has he conveyed away any of his property in the last four years, if so, what was it and to whom? Had none to convey. 14. What is the applicant's occupation and physicalcondition? trying to farm 15. Is the applicant unable to support himself by labor of any sort, if so, why? He is not because he is old and suffering from lung trouble. 16. How was he supported during the years 1898 and 1899? trying to farm 17. What portion of his support for these two years was derived from his own labor or income? Very little 18. Give full and complete statement of the applicant's physical condition that entitles him to a pension under section 1254, Code? old, infirm, unable to earn a living at any thing 19. AFFADAVIT OF PHYSICIANS State of Georgia Schley County Personally came before me J.N. Cheney, M.D. and __________ both known to be reputable physicians of said county, who, being severally sworn, say on oath that they have examined carefully T.J. Allen, applicant for pension under Section 1254, Code, and after such personal examination say that his precise physical condition is as follows: 64 years old, feeble and weak lungs. They further say on oath that the physical condition of applicant renders him unable to labor at any work or calling sufficient to support himself, and that we have no interest in said pension being allowed. J.N. Cheney, M.D. W.D. Sears, M.D. Sworn to and subscribed before me, this the 27th day of Oct. 1900. P.E. Taylor, Ordinary. ORDINARY'S CERTIFICATE State of Georgia Schley County I, P.E. Taylor, Ordinary in and for said county, hereby certify that the applicant T.J. Allen resides in said county, and has been a bona fide resident of this state since the (all his life) and that the witnesses viz: J.N. Cheney, MD & W.D. Sears, MD & M.W.Wimbish are of trustworthy character, and that before answering the foregoing questions the applicant and each witness took the oath hereon prescribed, and that the full text of the affadavits was read to the applicant and witness before same was signed. I further certify that the tax digests of Schley county show that applicant returned for taxation in his name in 1898, 10.00 dollars of property, and in 1899, 10.00 dollars of property. In my opinion, the foregoing claim is by him made in good faith. Witness my hand and seal of office, this 27th day of Oct. 1900. P.E. Taylor, Ordinary of Schley County. (Add'l Notes: A subsequent application for year 1906 reveals that he stated he had been a citizen and resident of the state of Georgia since 15 Dec 1837. Presumably this would have been his date of birth. Subsequent re-applications appear in the file for years thru 1907) WIDOW'S AFFADAVIT State of Georgia Schley County Personally before me comes Mrs. T.J. Allen of said county, who, after being duly sworn on oath says, that she is the widow of T.J. Allen to whom in the county of Schley, State of Georgia, she was married on the 14th day of Oct. 1860 and that she remained his wife, resided with him to the date of his death in January 1910 and that she has not since his death remarried. At the time of his death he was a resident of Schley county, in said stae of Georgia, and he was on the indigent pension roll of the state and paid a pension of $60.00 in Schley county for 1910 per annum, on account of being a soldier in Company G, 5th Regiment, Ga. Volunteers. At death of T.J. Allen he was in the use and possession of the following property: 101 acres land No. 131, one mule, 7sl(?) one covered wagon & buggy, household (unreadable) valued at $ 718.00. What property of any kind and of any value have you in your use, control and possession now, and the cash value? 101 acres of land valued at $450.00 1 mule valued at $75 1 cow valued at $25 Total cash value of all property $550.00 That she is now a bonafide citizen of said county of Schley and she has so continuously resided since 15 day of Aug 1842. Mrs. T.J. Allen Sworn to and subscribed before me, this the 20th day of Aug, 1910. P.E. Taylor, Ordinary of Schley County. (Add'l notes: Mrs. Thomas J. Allen was formerly Lucinda Jane Wall) AFFADAVIT OF WITNESS TO PROVEMARRIAGE AND TO WHOM--DATE OF DEATH OF HUSBAND State of Georgia Schley County Personally before me comes T.J. Myers, known to be responsible and truthful person, residing in said county, who after having duly sworn on oath, says: that of his personal knowledge, Mrs. T.J. Allen, who made the foreging affadavit, is the lawful widow of T.J. Allen, who died in Schley County in the State of Georgia on ______ day of ______ 1909 and that she has not since remarried. That she became the wife of T.J. Allen on the 11th day of Oct. of 1860 and that she and he had resided together as man and wife continuously since 11th day of Oct. 1860 and that the T.J. Allen was the same man who was on the pension roll of said state Ga. from Schley County when he died. T.J. Myers Sworn to and subscribed before me, this the 5th day of Nov. 1910. P.E. Taylor, Ordinary of Schley County. (Add'l notes: Marriage records for Schley County indicate the marriage date was 14 Oct 1860. T.J. Allen's marker at Hopewell Methodist cemetery indicates a date of death of Jan. 30, 1910. There is an affadavit of freholders T.J. Hixon and J.T. Royal attesting to the valuation of her property as stated above and an Ordinary's certificate affirming the above data. These documents repetitive and not transcribed. Thomas J. Allen was one of four brothers that enlisted in Co. G, 5th Ga. Inf. The others were Augustus E. (died of typhoid fever at Canton, Ms. during the war), Aurelius, Henry H., and William.)
