Jones of Schley County, State of Georgia, who, being duly sworn, says on oath that he is a bona fide citizen and resident of said state, and has been such since the 10th day of November 1842; that he enlisted in the military service of the Confederate States during the war between the states, and served as a private in company B, of 17th Regiment of Georgia Volunteers, Benning's Brigade; that whilst engaged in such military service, at Manassus in the state of Virginia, on the ___ day of Dec. 1861, he was wounded as follows: Left arm was wounded and amputated below the elbow by Dr. T.J. Rains, surgeon 17th Regt. Ga. Vol. Which wound permanently disbales deponent and renders nhim practically incompetent to perform manual labor, and his arm, or leg, or __________, substantially useless. Deponent desires to participate in the benefits of the Act, approved October 24, 1887, and makes application for the allowance to which he is entitled thereunder. E.M. Cheney, clerk, SCSC There is a handwritten note on nthe bottom of his power of attorney form that says: "send money by check to me at LaCrosse, Ga."
