Lindsay, who being sworn according to law, says under oath that she is a widow of J.Z. Lindsay, who was a soldier in the service of the Confederate States, and served as a member of Company H, of the 46th Regiment of (unreadable) Volunteers; that he enlisted in said service on or about the 15th day of April 1862, and was in the Tennessee Army up to August 1863. That while in the Army, he was on the _____ day of July 1863, that while in the service of the Confederate Army taken sick with cronic diarrhea, which disease(?) grew worse & was on or about the 1st of March, '64 sent from army to to hospital in Atlanta, Ga. & from said hospital was furloughed home 1st of April 1864 & on the 17th day of June 1864 (a space of six weeks) died of said disease. I positively know that he ddied of said disease (chronic diarrehea) which contracted while in service, I further swear that he was a sound man & in perfect health when he went into the army. Deponent further swears that she was the wife of said deceased soldier during his term of service in the army, and that she has never married since his death; that shebecame his wife on the 23rd day of December, 1858, and that she has resided in Georgia continuously since the 7th day of July 18?5; that Georgia is her home and was such on the 23rd day of December, 1890, and since said date she has not lived in any other state or locality. Deponent, as the widow of said deceased soldier husband, applies for the pension provided by Act of the General Assembly of Georgia, approved December 23rd, 1890, for the pension year ending February 15th, 1892, and herewith tenders the proof of her right to receive the allowance granted by said Act. State of Georgia County of Schley In person came before me, the undersigned Ordinary in and for said county, witneses J.M. Gay, G.W. Tison and ______________ (each known to said Attesting Officer as truthful, reliable and reputable citizens), who severally say under oath, that, from their own personal knowledge, Mrs. Flora F. Lindsay, of the County of Schley, State of Georgia, is the widow of J.Z. Lindsay, who was a soldier in Company H, 46th Regiment of Georgia Volunteers. That said soldier enlisted in the service of the Confederate States (or the Georgia State Troops) on or about the 15th day of April 1862. That while in said service, or by reason of said service in the army, he lost his life as follows: That while in the service of the Confederate Army, he contracted the diease of Chronic Diarrhea, which disease was extremely severe & was on or about the 1st of March 1864, sent from Army of Tennessee to Atlanta Hospital Ga. & from said hospital was furloughed home on the first of April 1864 ( space of sic or seven weeks), died of said disease, Chronic Diarrhea which he contracted while in service. We further swear that he positively died of said disease, he being a sound man & in perfect health at the time of entering the army. We further swear that Mrs. Flora F. Lindsay was the wife of said soldier during the service, and that she has not intermarried since his death, and that she resides in Schley County of the state of Georgia. J.M. Gay G.W. Tison M.B. Additional Comments: Add'l Info: John Zack Lindsay was married to Flora Cockrell in Schley County on 23 Dec 1858. Iverson L. Lindsay was their son. They are all buried at Ellaville Cemetery.
