Eubanks of said state and county, desiring to avil himself of the pension act approved December 15th, 1894, hereby submits his proofs, and after being duly sworn true answers to make to the ollowing questions, deposes and answers as follows: 1. What is your name and where do you reside? Amos H. Eubanks I reside in Schley Co., Ga., Ellaville P.O. 2. Where did you reside on January 1st 1894, and how long have you been a resident of this state? Resided in Schley Co., Ga. 65 years 3. When and where were you born? Dec. 17, 1830 in Schley Co. 4. Did you volunteer in the Confederate Army or Georgia Militia? Confederate Army 5. When and where did you enlist? Ellaville, Ga. on March 1862 6. In what company and regiment did you enlist? Co. B, 46th Ga. Regiment 7. How long did you remain in that company and regiment? till May 1865 8. If you were discharged from same and joined another, or if you were transferred to another, give an account of such discharge or transfer? never discharged or transferred 9. For how long a period did you discharge regular military duty? 3 years, liking 2 months 10. When, where and under what circumstances were you discharged from service? never discharged 11. What is your present occupation? Farming 12. How much can you earn per annum by your own exertions or labor? nothing 13. What has been your occupation since 1865? doing all I could to farm 14. What sum would be necessary for your support for this pension year, and how much are you able to contribute thereto either in labor or income? $100.00, possibly $35 0r $40 might contribute. 15. What is your present physical condition and how long have you been in such condition? I am diseased, palpitation of the heart & spinal effection & kidney effection. 16. Upon which of the following grounds do you base your application for pension, viz.: first, "age and poverty," second "infirmity and poverty" or third "blindness and poverty"? Infirmity and poverty 17. If upon the first ground, state how long you have been in such condition that you could not earn your support? If upon the second, give a full and complete history of the infirmity and it's extent? If upon the third, state whether you are totally blind and when and where you lost your sight? My infirmity is so located in my spinal column that I am unable to stoop or lift anything & my kidney effection gives me great trouble in making discharge of water fluids at times. 18. What property, effects or income do you possess? none 19. What property, effects or income did you possess in 1893 and in 1894 and what disposition, if any, did you make of same? none 20. In what county did you reside during those years and what property did you then return for taxation? In Schley Co., returned no property 21. How were you supported during the years 1893 and 1894? my own labor & help of my friends 22. How much did your support cost for each of those years, and what portion did you contribute thereto by your own labor or income? about $100.00. I probably contributed about $35.00 or $40.00. 23. What was your employment during 1893 and 1894? What pay did you receive in each year? I was trying to farm. $35 or $40. 24. Are you married and have you a family? If so, is your wife living and how many children have you? Give age and sex of children and their means of support? Yes. Six children, my wife is living. 2 boys & ages respectively 36 & 39, 4 girls, ages respectively 32 to 37. They have no means except by their own labor. 25. Are you receiving a pension under any law of this State, if so, what amount and for what disability? receiving no pension Amos H. Taylor, of said state and county, havingben presented as a witness in support of the application of Amos H. Eubanks for pension under the Act approved December 15th, 1894, and after being duly sworn true answers to make to the following questions, deposes and answers as follows: 1. What is your name and where do you reside? John T. Taylor, in Schley Co., Ga. 2. Are you acquainted with Amos H. Eubanks, the applicant, if so, how long have you known him? 40 years 3. Where does he reside, and how long has he been a resident of this State? in Schley County, Ga. Has been a resident to my knowledge 40 years. 4. Do you know of his having served in the Confederate army of the Georgia Militia? How do you know this? Yes, because I served with him. 5. When, where and in what company and regiment did he enlist? in Co. B, 46 Ga. Regiment March 2, 1862. 6. Were you a member of the same company and regiment? Yes 7. How long did he perform regular military duty, and what do you know of his service as a Confederate soldier, and the time and circumstances of his discharge from the service? he was discharge 3 years liking 2 months. I was with him his time and he was ?????. He made a good soldier. 8. What property, effects or income has the applicant? None. I am his neighbor & see him ever week or two. 9. What property, effects or income did the applicant possess in 1893 and 1894, and what disposition, if any, did he make of same? None 10. What is the applicant's occupation and physical condition? Farming. His condition, physical condition is very bad, being old, infirm, and afflicted. 11. Is the applicnt unable to support himself by labor of any sort, if so, why? Yes, because he is old, infirm and afflicted & very poor, having no property of any sort whatever. 12. How was he supported during the years 1893 and 1894? by his labor & contributions of others 13. What portion of his support for these years was derived from his own labor or income? I suppose as much as $35 of $40. 14. Give a full and complete account of the applicant's physical condition that entitles him to a pension under the Act of December 15th, 1894? He was a Confederate soldier, served forthrightly for 3 years liking 2 months, has no means of support & is so afflicted that he cnnot support himself at any work whatever or occupation of any kind. 15. AFFIDAVIT OF PHYSICIANS State of Georgia Schley County Personally came before me W.D. Sears and J.N. Cheney, MD, both known to me as reputable physicians of said county, who being severally sworn, say on oath that they have examined carefully Amos H. Eubanks, applicant for pension under the Act of 1894, and after such personal examination, say that his precise physical condition is as follows: Has spinal intation, indigestion & palpitation of the heart, feeble from old age & excessive hard labor, protracted cough of long standing. We further say on oath that the physical condition of applicant renders him unable to labor at any work or clling sufficient to earn a ful support for himself, and that we have no interest in said pension being allowed.
