English of said state and county, desiring to avial himself of the pension act approved December 15th, 1894, hereby submits his proofs, and after being duly sworn true answers to make to the following questions, deposes and answers as follows: 1. What is your name and where do you reside? Samuel H. English LaCrosse, Schley Co., Ga. 2. How long and since when have you been a resident of this state? all my life 3. When and where were you born? Houston County, Ga. 4. When and where and in what company and regiment did you enlist or serve? Enlisted 22nd July, 1864 at Oglethorpe, Ga. in Co. "G," 11th Ga. State Troops. 5. How long did you remain in such comapny and regiment? Until the close of the war. 6. For how long a period did you discharge regular military duty? 9 months. 7. When, where and under waht circumstances were you discharged from service? Discharged at close of war at Macon Ga. 8. What is your present occupation? Not able to do anything. 9. How much can you earn per annum by your own exertions or labor? Nothing. 10. What has been your occupation since 1865? Farming 11. Upon which of the following grounds do you base your application for pension, viz: first "age and poverty," second "infirmity and poverty" or third "blindness and poverty"? no answer. 12. If upon the first ground, state how long you have been in such condition that you could not earn your support? If upon the second, give a full and complete history of the infirmity and its extent? If upon the third, state whether you are totally blind and when and where you lost your sight? age & poverty & blindness 13. What property, effects or income do you possess and it gross value? Own no property. 14. What property, effects or income did you possess in 1894, 1895, 1896 and 1897 and what disposition, if any, did you make of same? Own no property in 1894, 1895, 1896 & 1897. 15. In what county did you reside during those years and what property did you then return for taxation? Reside in Macon Co. 1894, 95,96 & 97. Schley Co. 98. No proprty for taxation. 16. How were you supported during the years 1896 and 1897? By my friends. 17. How much did your support cost for each of those years and what portion did you contribute therto by your own labor or income? About 60.00, nothing by my own labor. 18. What was your employmen during 1896 and 1897? What pay did you receive in each year? Farming, received no pay. 19. Have you a family? If so, who composes such family? Give their means of support? Have they a homestead? Have a family, wife & two children, Farming, Have no homestead. 20. Are you receiving any pension, if so, what amount and for what disability? Receiving no pension. Samuel H. QUESTIONS FOR WITNESS State of Georgia Macon County Isaac Jessup and Thomas English of said state and county, having been presented as witnesses in support of the application of S.H. English for pension under the act approved December 15th, 1894, and after being duly sworn true answers to make to the following questions, depose and answer as follows: 1. What is your name and where do you reside? Isaac Jessup, I live in Macon Couny, Ga. 2. Are you acquainted with ____________ , applicant, if so, how long have you known him? I am, have known him more than 30 years. 3. Where does he reside, and how long and since when has he been a resident of this state? In Schley County, Ga. Has lived in state ever since I knew him. 4. When, where and in what company and regiment did he enlist, and how do you know? July 22nd, 1864 in Macon County Vols. 11th Ga. State Troops, was in same Co. 5. Were you a member of the same company and regiment? Yes 6. How long did he perform regular military duty, and what do you know of his service as a Confederate Soldier, and athe time and circumstances of his discharge from the service? About 9 months, was a good, average soldier, was discharged at close of the war. 7. What property, effects or income has the applicant? He has none, has lived near me all his life. 8. What property, effects or income did the applicant possess in 1896 an 1897, and what disposition, if any did he make of same? None. 9. Has he conveyed away any of his property in the last three years, if so, what was it and to whom? No. 10. What is applicant's occupation and physical condition? A farmer. He is old (73 years of age) and physically unable to make a support. 11. Is the applicant unable to support himself by labor of any sort, if so, why? He is by reason of age and infirmity. 12. How was he supported during the years 1896 and 1897? By what little he could do & the help of friends. 13. What portion of his support for these two years was derived from his own labor or income? Small portion, has no income. 14. Give a full and complete statement of the applicant's physical condition that enables him to a pension under the Act of December 15th, 1894? He is old, poor & unable to work. 15. What interest have you in the recovery of a pension by the applicant? Isaac Jessup testifies as to service and Thomas English as to remaining questions. J.S. Elmore, Ordinary, Macon County. AFFIDAVIT OF PHYSICIANS State of Georgia Schley County Personally came before me J.N. Cheney, MD and B.L. Bridges, both know to me as reputable physicians of said county, who being severally sworn, say on aoth that they have examined carefully Samuel H. English, applicant for pension under the Act of 1894, and after such personal examination say that his precise physical condition is as follows: 73 years old, worn out & defection eyesight. We further say on oath that the physical condition of applicant renders him unable to labor at any work or calling sufficient to earn a support for himself, and that we have no interest in said pension being allowed. J.N. Cheney, MD B.L. Bridges, MD Sworn to and subscribed before me, this the 23rd day of Sept. 1898. Phil E. Taylor, Ordinary. Subsequent refilings of the pension indicate residence "in said state continuously ever since 22nd day of March, 1825."
