State of Georgia Schley County Personally appears D.T. Parker of Schley county, State of Georgia, who, being duly sworn, says on oath that he is a bona fide citizen and resident of said state, and has been such since the 8th day of April, 1841; that he enlisted in the military service of the Confederate States during the war between the states, and served as a private in Company B, of 46th Regiment of Georgia Volunteers, ????'s Brigade; that whilst engaged in such military service, at the battle of Chickamauga, in the state of Georgia, on the 19th day of September 1863, he was wounded as follows: shot by a minnie ball in the top of head just at the mole (?) of the head, said ball ranging in until it lodged in the brain. Said ball passing through the skull & lodged on the brain which causes vertigo, deponent having to carry his head in an upright position all the time & said wound has impaired the health of deponent which wound permanently disables deponent and makes him practically incompetant to perform manual labor. Deponent desires to participate in the benefits of the Act, approved October 24, 1887, and the Act amendatory thereof, approved December 24, 1888, and makes application for the allowance to which he is entitled thereunder for the year ending October 26, 1889. COMMISSIONED OFFICER'S AFFIDAVIT State of Georgia Fulton County Personally came before me R.J. Redding of the county of Fulton, State of Georgia, who, being duly sworn, says that he was a commissioned officer in Company B, of 46th Regiment of Ga. Volunteers, and that deponent knows D.T. Parker, and that he received the wounds in the military service, as stated in his foregoing affidavit, and thatwounds permanently disables the said ____________ as stated by him in said affidavit. Deponent further states that said D.T. Parker is a bona fide citizen of this state and resides in Schley county. R.J. Redding Ex. Capt. Co. B, 46th Ga. Vols. State of Georgia Schley County Personally came P.F. Dixon, J.H. Bivins & G.W. Tison & T.J. Myers, citizens of Schley county, in said state, who, being duly sworn, say that they are acquainted with D.T. Parker and know that he received the wounds in the military service, as stated by him in the foregoing affidavit; that said wounds permanently disables applicant, as ststed by him; that said applicant is a bona fide citizen of this state, and resides in Schley county, and we are well satisfied that all the statements in this affidavit are true. State of Georgia Schley County Personally comes before me T.B. Myers, Ordinary of said county, C.H. Smith and R.T. Bivins, both known to me as reputable physicians of said county, who, being severally sworn, say on oath that they have carefully examined D.T. Parker and after such examination say that the applicant has been injured as follows: shot by a minnie ball in the top of the head just in the rear of the mole, said ball passing through & fracturing the skull bone making a hole about the size of a silver dollar, said wound producing spells of vertigo & otherwise impairing the health of applicant which wound permanently disables applicant and renders him practically incompetent to perform manual labor.
