Williams of Schley county, State of Georgia, who, being duly sworn, says on oath that he is a bona fide citizen and resident of said state, and has been such since the ______ day of June 1841; that he enlisted in the military service of the Confederate states during the war between the States, and served as a private in Company A, of 10th Regiment of Ga. Bat. Volunteers ?????'s Brigade; that whilst engaged in such military service, at the battle of Petersburg in the state of Virginia, on the 22 days of June 1864, he was wounded as follows: In the left leg. Ball entered the out side of the tibia about 4 or 5 inches above the ankle joint & fractured the fibula in his places (?), 4 inches of the same, coming out cutting all the muscles of the back part of the leg which ????? & caused contraction of the Tardo (?) Achiles & other muscles & causing applicant to walk with the ball of the foot having to use a stick to walk with. My leg is substantially & essentially useless. Deponent desires to participate in the benefits of the Act, approved October 24, 1887, and makes application for the allowance to which he is entitled thereunder. T.L. Murphy, citizens of Schley county, in said State, who being duly sworn, say that they are acquainted with T.L. Williams and know that he received the wounds in the military service, as stated by him in the foregoing affidavit; that said wounds permanently disables applicant, as stated by him; that said applicant is a bona fide citizen of this state, and resides in Schley county, and we are well satisfied that all the statements in his affidavit are true. Dozier, Ordinary of said county, J.N. Cheney and C.H. Smith, both known to me as reputable physicians of said county, who, being severally sworn, say on oath that they have carefully examined T.L. Williams and after such examination say that the applicant has been injured as follows: As set forth in his affidavit & that in our opinion he is rendered permanently & practically incompentent for the performance of ordinary manual avocations of life. Ball entered the outside of the tibula about 3 or 4 inches & also the ankle joint breaking the fibula in two places & cut all the muscles of the back part of his leg. The fractued partof the fibula coming out & sloughing of the muscles caused the Tarde Achilies & all the other muscles of the back partof the leg to be contracted & ???? his heel ??? is three inches which causes applicant to walk on the ball of his foot & toes, it being impossible for him to walk (unreadable)... los of the bone & contraction of the muscles & stiff ankle ???? applicant uses a stick to walk with & ????? walk only on the ball of his foot. ??? his leg is essentially & substantially useless.
