Womack of Schley county, State of Georgia, who being duly sworn, says on oath that he is a bona fide citizen and resident of said State, and has been such since the ..................... day of ................................ 1840; that he enlisted in the military service of the Confederate States (or of the State of ............................... ) during the war between the States, and served as a Private in Company G, of 5th Regiment of Ga. Volunteers, Jackson's Brigade; that whilst engaged in such military service, at the battle of Murfreesborough in the State of Tennessee, on the 31st day of Dec. 1862, he was wounded as follows: in the right wrist & arm, ball entering above the wrist japing(?) between the bones & out near the elbow cutting muscles & everything so as to render the wrist joint & elbow stiff. Deponent desires to participate in the benefits of the Act, approved October 24, 1887, and makes application for the allowance to which he is entitled thereunder. Snipes of the county of Schley, State of Georgia, who being duly sworn, says that he was a commissioned officer in Company G, of 5th Regiment of Ga. Volunteers, and that the deponet knows Chas. Womack, and that he received the wounds (or contracted the disease) in the military service, as stated in his foregoing affidavit, and that wounds (or disease) permanently disables the said ..................................................., as stated by him in said affadavit. Deponent further states that said Charles Womack is a bona fide citizen of this State, and resides in Schley county. Sworn to and subscribed before me, this 15th day of Feb, 1888. William Allen, N.P.S.C. James J. Snipes 2nd Lt. Co. G, 5th Ga. Vols. STATE OF GEROGIA} Schley County. Personally comes before me T.J. Dozier, Ordinary of said county, R.T. Bivins and J.N. Cheney, both known to me as reputable physicians of said County, who, being severally sworn, say on oath that they have carefully examined Chas. Womack and after such examination say that the applicant has been injured as follows: wounded in the right wrist, ball entered above the wrist joint & ?? upward & came out at the elbow japing between the bones cutting the muscles leaving him with a stiff wrist & elbow joint which to say renders him practically incapable to perform ?? manual labor. Dozier, Ordinary of said county, do certify that I am acquainted with Chas. Womack, the applicant in the foregoing affidavit, and am satisfied that the statements made by him in his said affidavit are true, and I know he is the individual he represents himself to be, and that he resides in this county. I also certify that the foregoing witnesses are persons of respectability, and that their statements are worthy of full credit and belief. I futher certify that J.N. Cheny and A. Allen before whom the foregoing affidavits were made and power of attorney was signed, is a clerk Supr Court & N Publ???? of said county, and that the said affidavits and signatures thereto are genuine. Know all men by theses presents, That I Chas. Womack of Schley county, in said State, do hereby appoint Capt. R.J. Reddy of Atlanta my true and lawful attorney in fact, for me and in my name to receive and receipt for what ever amount of money I may be entitled to from the State of Georgia by reason of the injury received as aforesaid in the military service of the Confederates States (or of this State), as stated in the forgoing Affidavit. Hereby authorizing my said attorney to receipt in my name for any Warrant that may be issued by the Governor, or for any sum of money which may be coming to me for the reason aforesaid. In witness whereof I have hereunto set my hand and seal, this 18th day of Feb, 1888. Chas. Womack [L.S.] Executed in the presence of us: J.N. Cheney, clerk Charles Womack's pension was initially $25. The amount was raised to $50 in 1889 and remained at that until his death in 1900. He reapplied each and every year. I can find no record of a widow's pension. On his 1891 application he lists his residency in the sate of Georgia as the 1st of March, 1836, which I expect would have been his date of birth. Of those who signed his pension applications, R.T. Bivins, William Allen, A(urellius) Allen were all members of Co. G, 5th Ga. as was Robert Burton who subsequently signed an affadavit. Capt. Burton was 1st Captain of Co. G.
