Account to be made by Quartermaster I certify, that the within named John Forshee, a Private of Captain WS Wallace, Company E of the 45th Regiment of Georgia Vol., born in Houston County in the State of Georgia, aged Twenty five years, five feet, 10 inches high, fair complexion, grey eyes, sandy hair, and by occupation a farmer was enlisted by Capt. Wallace at Butler on the 4th day of March 1862, to serve three years, and is now entitled to discharge by reason of a Surgeon's certificate of disability. The said John Forshee was last paid by Capt. John T. Brown to include the 28th day of February 1863, and has pay due from that date to the present date. There is due him Sixty three 24/100 dollars for clothing. He is indebted to the Confederate States nothing 00/100 dollars. Given in duplicate at Camp Gregg, this 3rd day of April 1863. WS Wallace, Capt.
