(Jim) Carroll williamcarrollj@bellsouth.net October 28, 2010, 3:40 pm Disability-Application For Allowance Twiggs County, State of Georgia William Chance states that he is a citizen of said state and has been since 17 Sep 1836, that he enlisted in military service in the CSA as a Private in Company I of the 6th Regiment of Georgia Volunteers and while engaged in miltary service, at the battle of ____ up at Petersburg in the state of Virginia in July 1864, he was wounded by a grape shot entering the right shoulder, fracturing the shoulder blade and collar bone and lodging in right lung. Said wound producing asthma of a much aggravating kind and labored breathing together with shortness of breath as to render applicant substantially and essentially incapable of normal labors. Also, the right arm has since been emaciated and helpless to a degree to render it substantially and essentially useless. His Wm (X) Chance Mark T. M. Johnston, Clerk Witnesses supporting his affidavit were J. T. Horne, John Glover, & W. R. Martin. Physicians Thomas S. Jones and J. M. Wall examined William Chance and signed his Medical Affidavit before J. R. Wimberly, Ordinary. Power of Attorney was given to Hon. W. A. Wright of Atlanta, Ga. by William Chance and instructed to send money to him via registered letter to Jeffersonville, Twiggs County care of Ordinary. NOTE--Similar applications are filed in 1890, 1891, 1892, 1893, 1894, 1896, 1897, 1898, 1899, 1900, 1901, 1902, 1903, 1904, 1905, 1906, 1907. All of these were for $50.00.
