Ford Co. K, 5th GA Reg't, Georgia Vols., C. S. A. Camp No. 1480 Filed Feb. 7, 1905 Feb. 7, 1905 To The United Daughters Of The Confederacy: The undersigned, residing at The Rock, Upson Co. Ga. Who is an ex-Confederate soldier, and a member of Camp, No. 1480 U.C.Vs., hereby, at your request, presents this Certificate of Eligibility for a Confederate Cross of Honor. He entered the service of the Confederate States on the 7th day of May 1861, as a Private in Company K of the 5th Ga. Reserves Regiment of the Upson CountyVolunteers. And at that time was a Resident of UPSON CO. He was honorably discharged from service by Surrender of J. E. Johnson Army, on the 26th, Day of April 1865, at which time he held rank of PRIVATE.
