The Doctor is fearful that an embryo cancer is making its appearance on his under jaw, hence he goes to Philadelphia to consult the best medical practitioners in his case. It is not likely that he will ever practice medicine again in Bellevue, as he contemplates on his return to locate in larger city and leave us altogether.
