Army of the United States. Certificate of Disability for Discharge.
George W. Haworth, a Private of Captain H.P.W. Cramer's Company (A), of the Fiftieth Regiment of Illinois Inf., was enlisted
by Edgar Picket, of the (blank) Regiment of (blank) at Marcelline, Ill., on the 20th day of August 1861, to serve 3 years; he
was born in (blank) in the State of Ohio, is eighteen years of age, five feet five inches high, light complexion, blue eyes, dark
hair, and by occupation when enlisted a Farmer. During the last two months said soldier has been unfit for duty 60 days.

I certify, that I have carefully examined the said George W. Haworth, of Captain Cramer's Company, and find him incapable of
performing the duties of a soldier because of Gunshot wound received at the battle of Corinth on 3 of October. Ball entered on
radial side of right forearm in its middle, causing a compound fracture of radius and ulna. The fracture is united and the man will
have full use of his arm in the course of four to five months. (In different handwriting - No case for pension (signed)

Claim for Invalid Pension State of Illinois Adams County ss.

On this twenty seventh day of June 1863, before me, the undersigned a Judge of the County Court in and for the County and
State above named, personally appeared George W. Haworth a resident of Honey Creek Twp in the State of Illinois aged 20
years, who being duly sworn according to law, declares that he is the identical George W. Haworth who was a private in
Company A commanded by Captain Cramer in the 50th Regiment of Ills Volunteers, commanded by Colonel M. M. Bane, in
the war of 1861 and 1862, for the suppression of the Rebellion. That he volunteered at Marcelline, in the State of Illinois on or
about the 20th day of August 1861, for the term of three years and was honorably discharged at St. Louis Mo on the 21st day
of January 1863, as will appear by his certificate of discharge herewith presented.

That while in said service in the line of his duty at Corinth Miss. In the State of Mississippi on the third day of October 1862 he
received the following would to wit --: A gun shot - through the right-arm causing a compound fracture of the radius and ulna
causing stiffness of the arm and fingers and he claims one half disability and a pension for the (cannot read).

He makes this declaration for the purpose of being place on the Invalid Pension Roll of the United States by reason of the
disability above stated; and hereby constitutes and appoints C____ Meely of Quincy Ills, his attorney to prosecute this claim
and procure as Pension Certificate. (signed) George W. Haworth

Sworn to, subscribed and acknowledged before me, the day and year first above named, and on the same day personally
appeared Thomas C. White and James Haworth residents of Adams Co. Ills, and made oath that they are personally
acquainted with George W. Haworth who has made and subscribed the foregoing declaration in their presence, and that they
have every reason to believe from the appearance of the applicant, and their acquaintance with him, that he is the identical
person he represents himself to be, that they reside as above stated, and are disinterested in this claim for a Pension.

Officer's Certificate to Disability of Soldier
State of Mifs, County of Tishomingo ss.
I, Henry P.W. Cramer do hereby certify that I am Captain of Company A, of the 50th Regiment of Illinois Infty Volunteers,
and am acquainted with George W. Haworth, who was a member of my Company, and as I am informed is an applicant for an
Invalid Pension. That the said George W. Haworth was mustered into service on or about the 12th day of September, A.D.
1861, and discharged for disability about the 21st day of January 1863, having become disabled from doing duty as a soldier
from on or about the 3rd day of October A.D. 1862, while in the service of the United States, and in the line of his duty as a
soldier, in the manner and at the place as follows had the cords of his arm cut between the wrist and elbow by A gunshot, while
in an engagement with the enemy at Corinth Mifs on the 3rd Day of October 1862.

That the said soldier was in good health at the time he entered the service, and the disability above referred to affected him
while in the service, and at his discharge, as follows: rendering him unfit for Duty as A Soldier. Corinth Mifs Aug 28th 1863

Sir: I have the honor to acknowledge the receipt from your Office of application for Pension No. 97,596, and to return it
herewith, with such information as is furnished by the files of this Office.

It appears from the Rolls on file in this Office, that Geo. W. Haworth was enrolled on the 24 day of Feby., 1864, at Quincy,
Ills. In Co. "A", 50 Regiment of Ills. Volunteers, to serve 3 years, or during the war, and mustered into service as a Pvt. & Vet.
Vol. From the 24 day of Feby. 1864, at Quincy Ills, in Co. "A", 50 Regiment of Ills. Volunteers, to serve 3 years, or during the
war. On the Muster Roll of Co. "A" of that Regiment, for the months of Sept. and Oct. 1864, he is reported "In Hospt.
Wounded Oct. 5/64." "Mustered Out with Co. - July 13/65 at Louisville, Ky." (This man was first enlisted in this Co. Aug.
20/61. - Was wounded Oct. 3/62. Dischgd. At St. Louis Jany. 1/63) - after which he reenlisted as above stated.

I John S. Worman being first duly sworn according to law say that I was Captain of Co. A. of the 50th Illinois Inftry in
command thereof on the 5th day of Oct. 1864. And while George W. Haworth who was a private at said time in said Co. was
on duty in the Battle Alatoona Ga. On said 5th day of Oct 1864 he the said George W. Haworth was wounded by a gunshot
from the enemy which struck and entered his left cheek close to and a little above the corner of his mouth and came out on the
other side right under his right ear.

And I further certify that said wound received as aforesaid rendered said Haworth unfit for military duty and he was therefore
sent to the Regimental Hospital then to Chatanooga Hospital & to Nashville Hospital - then to Evansville Hospital Ind. & finally
to Quincy Illinois & discharged on the (date not clear) day of July 1865.

And that said Haworth is & was a man of good habits. And I further state that all of the above facts 7 Statements are made
from the Company Record. And that I have no interest whatever in this claim or in its prosecution. (signed) J. S. Worman

State of Iowa Polk County ss.

On this 2nd day of Sept 1875 before me J. H. McClelland, Clerk of the Circuit Court in & for the county of the aforesaid
personally came George W. Hawoth aged 32 year, a resident of Hartford, Warren County, Iowa who being duly sworn
according to law claims that he is a pensioner of the United States, duly enrolled at the Des Moines Iowa Pension Agency at
the rate of Four dollars per month, -- then on file at 8. (?) per month.

By reason of disability incurred in the military service of the United States while a Pv in Co H Regt. Illinois Infy vols War of
1861. That his present physical condition is such that he believes himself entitled to receive an increased pension of at least 12
(?) a mo. & that he herewith returns his present pension Certificate No 79063 dated 23 January 1868.

He further declares that he is disabled in the following manner - Viz

Gun shot wound entering the left cheek near the mouth and coming out at the right ear injuring very badly the jaw bones -
infact, destroying them & hindering him from taking & eating to a great extent. That to talk understandingly he is obliged to
keep cotton in his mouth. That said wound occurred at Altoona Georgia on the 5 October 1864.

He hereby constitutes & appoints Wm. H. Leus of Des Moines Iowa his true & lawful attorney to prosecute this claim. He says
his residences is in Warren County Iowa & his P.O. Hartford. (signed) George W. Haworth

Also personally appeared at the same time & place Thomas J. Wright Junr, aged 44 years a resident of Warren Co Iowa &
whose P.O. is Palmyra and Nathan M(or H)errit aged 45 years, a resident of Warren Co Iowa & whose P.O. is Palmyra.

Persons whom I certify to be respectable & entitled to credit & who being by me duly sworn according to law say that they
were present and saw Geo. W. Haworth the claimant sign his name to the foregoing declaration & Power of Attorney. And
(can't read) said acquaintances & the appearance of the said applicant declare that he is the identical person he represents

On this 7 day of February, A.D. one thousand eight hundred and 90 personally appeared before me, a Notary Public within
and for the county and State aforesaid, George W. Haworth, aged 46 years, a resident of the Township of Richland, county of
Warren and State of Iowa, who, being duly sworn according to law, declares that he is a pensioner of the United States,
enrolled at the Desmoines Iowa Pension Agency at the rate of 14 dollars per month, by reason of disability from gunshot
wound in the left side of face and right bone arm and rheumatism in said arme incurred in the military service of the United
States while in A private in company A 50th Ills Inft. That I believe myself to be entitled to an increase of pension on account of
gun shot in fact and arm. I claim increase on account of my teeth which has gave way within the last year and I allso claim that I
never was rated high enought. That my post office address is Hartford county of Warren, State of Iowa.

On this 18 day of July, A.D. one thousand nine hundred and 1908, personally appeared before me, a Notary Public within and
for the county and State aforesaid, Clara C. Haworth, aged 58 years, a resident of Carlisle Iowa, county of Warren, State
Iowa of, who, being duly sworn according to law, makes the following declaration in order to obtain pension under the
provisions of the Act of Congress Approved April 19, 1908.

That she is the widow of George W. Haworth, who was Enrolled under the name of George W. Haworth, at Springfield
Illinois, on the 19 day of January, 1863, as a private in co.A 50 Ill. Vol. Infantry, and honorably discharged 21st of January
1863, having served ninety days or more during the late civil war.

That he also served Co.A 50th Ill. Inf. Regiment was enrolled on the 24th day of Feb. 1864 Discharged 13th day of July 1865
at Louisville Kentucky

That he was not in the military of naval service of the United States otherwise than as stated above.

That she was married under the name of Clara C. Robertson to said solider at East Des Moines Iowa, on the 20' day of
December, 1883, by A. H. Shafer; that there was no legal barrier to the marriage; that she had not been previously married;
that the soldier had been previously married, to Mary E. Reese 16 day of November 1865. Mary E. Haworth who died
January the 27 1882 and that neither she nor said soldier married otherwise than as stated above.

That the said soldier died July 31st, 1908, at Carlisle Iowa; that she was not divorced from, and that she has not remarried
since his death. Not

That the said soldier left the following-named children who are now living and under sixteen years of age, to wit: (blank)

That she has not heretofore applied for pension.

relation to aforesaid claim as follows:

That we were personally acquainted with George W. Haworth and that he died on the 31st day of July 1908.

We also state that we was personally acquainted with Clara C. Robertson who was married to George W. Haworth Dec. 20th
1883 and lived with him until his death July 31 - 1908

George W. Haworth was married to Mary E. Rees November 16th 1865 and lived with her until her death January 27th 1882

Des Moines, County of Polk, State of Iowa and Mary Haworth, aged 58 years, a resident of 1614 E.Grand Ave. street,
County of Polk, State of Iowa, well known to me to be reputable and entitled to credit, and who, being duly sworn, declare, in
relation to aforesaid case as follows: That we were personally anwell acquainted with George W. Haworth/Clara C. Robertson
and that they were married at our home and that we were present and witnessed the ceremony: which occured at 8 o.c. in the
evening Dec.20'1883

We also declare in relation to the above case that we have known the aforesaid George W. Haworth and Clara his wife all
