This is to Certify That* FORREST D. STOREY    CC9-323018

a member of the CIVILIAN CONSERVATION CORPS, who was enrolled  

October 10/1940  at  Jerome, is hereby 

DISMISSED therefrom, by reason of** Desertion  43a  ccc Regulations War

 Department 1937
   
  Said Forrest D. Storey  was born in Swinton 

in the State of Missouri    When enrolled he was twenty one years of age

 and by occupation a New worker   He had blue eyes,  Sandy hair,  

Light complexion, and was Six feet one inchs in height.  His color was White

  GIVEN UNDER MY HAND AT  Camp S-223 McCall, this Eighteenth day of June,

one thousand nine hundred and Forty one
