He had fired two shots and was firing the third when an explosion occurred, killing him instantly. A miner who worked in the next room was showing him a light from behind a loaded car; he stated that when Mr. Lofy put his lamp to the squib that he did not leave it as soon as it ignited and that he apparently lighted the squib near the powder and not at the extreme end of the match.
