Stevens was working in a room about four feet in that was being turned from another room, at about twenty feet back from the face. At the time of the accident the person working in the straight room and himself were each firing a shot. Stevens gave the word to fire and ignited the squib of his shot, which immediately exploded, causing injuries from which he died two days later. He left a widow and two children not dependents.
