Certificate of Disability for Discharge Lockwood John Lockwood John of Captain Cyrus N. Baird's Company, (E) of the 129th Regiment of United States Volunteer Infantry was enlisted by Cyrus N. Baird of the 129th Regiment Ill Vol. at Fairbury Illinois on the 8th of August 1862 to serve 3 years; he was born in Green County in the State of Pennsylvania is thirty Eight years of age, 5 feet 101/2 inches high, Dark complexion, Gray eyes, Black hair, and by occupation when enlisted a Farmer. During the last two months said soldier has been unfit for duty 60 days. Station: Fountain Head Date: Jan. 28th 1863 Cyrus N. Baird, Commanding Company I certify, that I have carefully examined the said Lockwood John of Capt. C. N. Baird's Company, and find him incapable of performing the duties of a soldier because of Chronic Nephritis, which followed Typhoid Fever. He has Edema of the lower extremities, is greatly debilitated and will not recover sufficient health to do the duties of a soldier. Baird's Company (E) of the 129th Regiment of Illinois Volunteers, of the State of Illinois, born in Greene Co, State of Pennsylvania, aged 38 years; 5 feet 101/2 inches high; Dark complexion, Hazel eyes, Black hair, and by occupation a Farmer, having joined the company on its original organization at Fairbury Illinois, and enrolled in it at the muster into the service of the United States at Pontiac Illinois on the Eighth day of September, 1862 for the term of Three Years and having served honestly and faithfully with his Company in the field to the present date, is now entitled to a DISCHARGE by reason of Death from Quincy. The said Lockwood John was paid by Paymaster Major ?. W. Tell to include the 8th day of September 1862 and has pay due him from that time to the present date; day of September 1862 and has pay due him from that time to the present date; he is entitled to pay and subsistence for TRAVELING to place of enrollment and what- ever other allowance are authorized to volunteer soldiers, or militia, so discharged.
