On this 15 day of September, 1887, personally appeared before me, a clerk of a court of record in and for the county and state aforesaid Martha Ann Misner, a resident of Johnson (Twp) in the county of Clark and the State of Ill. aged 59 years, who being duly sworn, makes the following declaration, in order to obtain the Pension provided by the act of Congress approved July 14, 1862. That she is the widow of Chas. W. Misener who was a private in Company F, commanded by............. in the 54th Regiment of Illa in the war of 1861; that her maiden name was Martha Ann Cooley and that she was married to Chas. W. Misener on or about the (3) day of November 1846, at Porterville, in the County of Crawford and State of Ill. by McSkimens, JP. and that she knows of no record of said marriage. SHE FURTHER DECLARES that said Chas. W. Misener her husband, died at Johnson Twp, Clark County in the State of Ill. on or about the 8th day of December 1875, of jaundice and general disability contracted while in the line of duty in the service of the United States. She also declares that she has remained a widow ever since the death of said Chas. W. Misener and that she has not in any manner been engaged in, or aided or abetted the rebellion in the United States; and she hereby appoints W. H. Wilest, Co. of Washington D.C. as her lawful attorney with power of substitution, and authorizes him to present and prosecute this claim. The following is the name, date of birth, and place of residence of all the children of her deceased husband who were under 16 years of age at the time of his death, Lidea Ellen Misner, Noah P. Misner, Clement L. Misner, Martha Addeline Misner, residence and post office address Moonshine, Clark Co., Ill. My Post Office address is Orange, Clark Co., Ill. On this Third(3) day of September A.D. 1888 personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Wm Staley, aged 62 years, a resident of Moonshine P.O., County of Clark, State of Illinois who, being duly sworn according to law, states that he is acquainted with Noah P., Lyda E., Clement E. and Martha A Misner, children of Martha A. and Charles W. Misner and knows that all lived to be over sixteen(16) years old. These facts are known affiant by his intimate acquaintance with above named persons. In the matter of Martha Ann Misener, widow of Charles W. Misener, Private Co.F., 48, Ill's, Inf. war 1861. This 12 day of November A.D. 1887, personally appeared before me a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Martha A. Misener, aged 59 years, who Post Office address is Moonshine, Clark County, Illinois, who being duly sworn, declared in relation to said case as follows: The reason why I do not furnish evidence of Regimental Surgeon who treated my husband while in the army, also, of his Commanding officers is, I do not know who they were and whether living or dead. The reason why I do not furnish evidence of the Physician who treated him after his return from the army till a short time before his death is has removed to California and I cannot find out his address. Report says he is dead. The reason why I do not furnish affidavit of births of children is this, I did not have physician called when either of them was born. I had a woman commanly called a midwife called in each case. She is dead. The Notary sends a certified copy of births of all my children taken from our Family Bible which is correct. Signed John W. E. Snider aged 48 years, a resident of York Township, in the County of Clark, and State of Illinois, who being duly sworn according to law, state that he was acquainted with Charles W. Misener applicant for Invalid Pension; and knows the said Chas. W. Misener to be identical person of that name who enlisted or volunteered as a Private in Company F. 48 Regiment of Ill's Vols, and who was discharged at Little Rock, Ark on or about 15 day of August 1865 by reason of Expiration of servce under, Circular 30. That the said Charles W. Misener while in the line of duty, at or near Newbern, North Carolina in the State of N.C. did, on or about the tenth day of February 1865, become disable in the following manner, viz: By extreme exposure on transport and marching he took a severe cold which caused a swelling of the body which terminated in jaundice and a general complication of disease from which he never fully recovered. I know these facts by being a member of and with the command of Chas. W. Misener when he contracted his disease. It was treated in General Hospital Ward at Newbern, NC. by Dr A. Coagle whose present address I do not know. After being discharged at Little Rock at same time he was, we came home together. Misener being so sick I and other Comrades had to assist him in reaching home. He had to procure medicine on the way home at Terre Haute, Ind. *********************************************************************** PROOF OF DISABILITY SATE OF Illinois, County of Clark SSgt; ON THIS eleven day of October A.D. 1887, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, John W. Burres, age 47 years, a resident of Johnsen Township in the County of Clark, State of Illinois, who, being duly sworn according to law, states that he was acquainted with Charles W. Misener applicant for Invalid Pension; and knows the said Chas. W. Misner to be the identical person of that name who enlisted or volun- teered as a Private in Company F., 48 Regiment of Illinois Vols., and who was discharged at Little Rock, Ark, on or about 15 day of August 1865, by reason of expiration of service under Circular 30. That said Charles W. Misener while in line of duty at or near Newbern in the State of N.C. did, on or about the tenth day of February 1865, become disabled in the following manner, viz: By extreme exposure while marching and camping during the early part of the Winter of 1865, he contracted jaundice and became diseased generally. He was treated in hospital at Newbern, North Carolina, I do not remember name of surgeon who treated him. I was a member of the Company and Regiment to which Misener belonged, I was with him in the hospital. When I was able to leave the hospital he was still there. I have been intimateley acquainted with Misener's family since the year 1857, living within one mile of them. I knew Chas. W. Misner from above date until his death. He went to the army a healthy man. He returned diseased as above stated, never fully recovered, dying December 8, 1875. His widow, Martha A. Misener, has not remarried since his death. I know these facts by my intimate acquain- tance as above stated. *************************************************************************** PHYSICIAN'S AFFIDAVIT. STATE OF Illinois, County of Clark SS In the Pension Claim No. Charles W. Misner late of Co. F., 48 Reg, Ill Vol (Private). Personally came before me a Notary Public in and for the aforesaid County and State; A. F. West, MD. a citizen of Casey, County of Clark, State of Illinois, well known to me to be reputable and entitled to credit, and who being duly sworn, declare in relation to the aforesaid case as follows: That he is a regular practising physician of 27 years standing, and has given medical advice and treatment to Chas. W. Misner as follows: I was call to see Charles W. Misner, Dec.the 8, 1875, the day he died. I found him in a comata state from which he never rallied. From his symptoms in that condition, and his general appearance, I am fully convinced that he died from jaundice; and from what I learned from his neighbors that day that I was called to see him, proved that my diagnosis was correct. They stated that he had had the jaundiced appearance more or less ever since he returned from the army and further he had not see a well day since he was discharged from the U.S. Service.
