Stephens, aged fifty eight years, a resident of R.R.No.1, Annapolis, County of Crawford, State of Illinois, who being duly sworn according to law, makes the following declaration in order to obtain pension under the provisions of the Act of Congress approved April 19, 1908. That she is the widow of John L. Stephens who was enrolled under the name of John L. Stephens, at...... on the 3rd day of April, 1865 as a private in Co. H, 6th Regt, Ill, Vol. Cav., and honorably discharged November 5th, 1865, having served ninety days or more during the late civil war. That he was not in the military or naval service of the United States otherwise than stated above. That she was married under the name of Susan P. Misner to said soldier at Felix Chesser's house on the 22nd day of July, 1876 by Felix W. Chesshir; that there was no legal barrier to the marriage; that she had not been previously married; that the soldier had not been previously married and that neither she nor said soldier married otherwise than as stated above. That the said soldier died July 26, 1913, at his home near Annapolis, R. D., That she was not divorced from him, and that she has not remarried since his death. That she has not heretofore applied for pension but deceased husband was pensioned under Ctf. No. 856997 That she hereby appoints, with full power of substitution and revocation, Edwin C. Madison, of Marshall, State of Illinois her true and lawful attorney to prosecute this claim. Her post office address is R.R.1, Annapolis, Crawford Co. Illinois. Signed Susan P. Stephens, the claimant, sign her name to the foregoing declaration; that they have every reason to believe from the appearance of said applicant and their acquaintance with her of 20 years and 30 years , respectively, that she is the identical person she represents herself to be; and that they have no interest in the prosecution of this claim Subscribed and sworn to before me this 13th day of October A.D. 1913, and I hereby certify that the contents of the foregoing declaration etc., were fully made known and explained to the applicant and witnessed before swearing and that I have no interest, direct or indirect, in the prosecu- tion of this claim.
