That he was never employed in the military or naval service of the United States after the 21st day of Sept, 1865, that she (was) married (as) Ellen Jenkins to said Levi Brady, on the __________ day of ________ A. P. at Green County, Indiana, there being no legal barrier to such marriage; that she had not been (the "she had not been" is underlined) previously married; that her said husband had been previously married. That his former wife died in the State of Ohio. That she has not remarried since the death of the said Levi Brady. That she is without other means of support than her daily labour and an actual net income of $250 per year. That the names and dates of all of their children, now living and under sixteen years of age: (none entered) That no prior application for pension has been filed by herself, the soldier was a pensioner by Cert #639.827, Act of June 27th 1890. That she makes this declaration for the purpose of being placed on the pension-roll of the United States, under the previsions of the acts of June 27, 1890 and May 9, 1900. Affiant says further that the discrepancy in her name as appears in the marriage certificate and the way she has signed her full name was Sarah Ellen, but she has never been called anything but Ellen from her girlhood up & she supposed that Ellen was the only name that was mentioned in the proceedings of the marriage license, but that it a . . . (On the margin of the affidavit it says affiant further stated that her late husband left no will as there was no estate, and that there was an insurance of ($50.00) fifty dollars on the life of her said husband & was paid her after his death.
