To be executed before a court of record or some officer thereof having custody of its seal, or before a notary public justice of the peace or any officer having an official seal, and authorized to administer oaths for general purposes. State of Indiana, County of Elkhart, SS: On this 4th day of August, A. D. one thousand eight hundred and ninety - personally appeared before me, a Notary Public within and for the County and State aforesaid, Anna M. McFarren aged 44 years, a resident of the city of Elkhart County of Elkhart, State of Indiana, who being duly sworn according to law, declares that she is the widow of James A. McFarren, who enlisted under the name of Jas. A. McFarren, at Indianapolis, Ind. on the 9th day of October, A. D. 1863, in 22nd Battery Light Artillery, Ind. Vols. and served at least ninety days in the service of the United States in the late War of the Rebellion, who was honorably discharged Indianapolis, Ind. and died March (May?) 19, 1882 - The cause of death need not be stated. That she was married under the name of Anna M. Sager to said James A. McFarren on the 1st day of August, 1872, by Revd John Biddle, at Elkhart Ind., there being no legal barrier to said marriage - None.  If there was a former marriage of claimant or her husband, state it here and how dissolved. That she has not remarried since the death of the said James A. McFarren. That she is without other means of support other than her daily labor; that names and dates of births of all the children now living under sixteen years of age of the soldier are as follows: Victoria M., born July 4th, 1875 Daisy L., born Sept. 12th, 1877 James A., born Oct. 6th, 1881 That she has heretofore applied for pension and the number of her former application is 344717. Now asks that said testimony of former claim be applied to this. That she makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. She hereby appoints, with full power of substitution and revocation, Martin Thomas her true and lawful attorney to prosecute her claim, and to receive therefor a fee of ten dollars; that her post-office address is Elkhart Ind.
