Johnson County Indiana From the Original Document, National Archives, Washington, DC: ACT OF MAY 11, 1912 DECLARATION FOR PENSION STATE OF Indiana, COUNTY OF Johnson, ss: On this 10 day of July, A.D. 1918, personally appeared before me, a Justice of the Peace within and for the county and State aforesaid, Isaac Settles who, being by me duly sworn according to law, declares that he is 73 years of age; that he was born on Dec. 22, 1844, at Needham, Ind.; that he is a resident of Needham county of Johnson, State of Indiana; and that he is the identical person who was enlisted in the service of the United States at Franklin, Indiana, under the name of Isaac Settles, on the 3rd day of May, 1864, as a Private in Co. G, 132 Regiment of Indiana Vol. Inft. and was discharged Sept. 7, 1864, at Indianapolis, Indiana. That he also served in Co. E, 148 Regiment of Indiana Vol. Inf, was enrolled Feby 18, 1865 and was discharged Sept. 5, 1865. That he was not in the military or naval service of the United States otherwise than as stated above. That his personal description at time of first enlistment was as follows: Height, 5 feet, 10 inches; complexion, Light; color of eyes, grey; color of hair, Light; that his occupation was Farmer. . . . That since leaving the service he has resided at Johnson County, Indiana and his occupation has been Farmer. . . . That he is . . . a pensioner under Certificate No. 478.350. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912. (Claimant's signature in full.)
