	The following is a transcript of the death certificate/death record of 
Jerry Cokonougher.  Jerry’s mother is not listed on the death certificate, but 
is believed to have been Mary Alice Pertee, his father’s first wife.  My 
cousin, Ena Cokonougher Mulford, who lives in Michigan, sent me this death 
certificate.
