D. one thousand eight hundred and ninety one personally appeared before me, a Notary Public within and for the County and State aforesaid, Henry Cozad aged 64 years, a resident of Red Cloud County of Webster State of Nebraska, who, being duly sworn according to law, declares that he is the identical Henry Cozad who was ENROLLED on or about the 12th day of August, 1862, in Co "H", 83rd Ill Infy but immediately made Drum Major 83 Ill Inf in the service of the United States in the war of the rebellion, and served at least ninety days, adn was HONORABLY DISCHARGED at Fort Donaldson, Tenn on the latter part or days of March, 1863. Also served no other service. That he is totally unable to earn a support by manual labor by reason of chronic indigestion - chronic diarrhea & alternate constipation. Piles, internal & external whil in the service & resulting Ulcers - lumbago or constant pain extending to lower end of spine . Chronic bronchitis with constant cough during winter months. Rheumatism in arms & limbs. partial blindness of both eyes from cataract and General debility. That said disabilites are not due to his vicious habits, and are to the best of his knowledge and belief, or a permanent character. That he has not heretofore applied for pension. That he has not been in the military or naval service of the United States since the said day of March 1863. That he makes this declaration for the purpose of being placed on the pension-roll of the United States under the provisions of the Act of June 27, 1890. he hereby appoints T W Tallmadge, of Washington, D. D. one thousand eight hundred and ninety-nine, personally appeared before me, a Notary Public, within and for the County and State aforesaid, Mary Cozad, aged 73 years, a resident of Long Island County of Phillips, State of Kansas who, being duly sworn according to law, declares that she is the widow of Henry Cozad, who enlisted under the name of Henry Cozad, on the 12 day of August A. D. 1862, as a Principal Musician in Company, in the 83 Regiment of Illinois Infantry Volunteers, and served at least ninety days in the late War of the Rebellion, in the service of the United States, who was HONORABLY DISCHARGED March 10, 1863, and died September 18th 1899. That he was not employed in the military or naval service otherwise than as stated above. That he was never employed in the military or naval service of the United States after the 10th day of March, 1863. That she was married under the name of Mary Tuttle to said Henry Cozad, on the 9th day of May A. D. 1848, by Rev L Stright at Mercer Co Pennsylvania, there being no legal barrier to such marriage; that she had not been previously married; that her said husband had not been previously married. That she has not remarried since the death of the said Henry Cozad. ........ That a prior application for pension has been filed by the soldier and he was pensioned on Certf No 842,823. That she makes this declaration for the purpose of being placed on the pension-roll of the United States, under the provisions of the act of June 27, 1890. That said soldier was my brother and I know he was not married previous to his marriage to Claimant. That I was present at the wedding of the Soldier & Claimant in Mercer Co Pennsylvania on the ninth day of May 1848 and was a witness to the ceremony of the marriage. That I was well acquainted with both Claimant & Soldier from said marriage till he Died on Sept 18 1899 and personally know that Soldier & Claimant lived together as Husband and wife from the date of said marriage to the date of his Death. Also that Claimant has not remarried Since the death of Soldier but is still his widow. D. 1899, personally papeared before me a notary public in and for the aforesaid County, duly authorized to administer oaths J M Carnaham aged 45 years resident of Long Island County of Phillips State of Kansas well known to me to be reputable and entitled to credit, and who being duly sworn, declared in relation to aforesaid case as follows: I am well and intimately acquainted with the Claimant above named and with her Condition financially. That said Claimant has no money, no stocks, nor bonds, nor investments of any Kind and no income or means of Support, now is any person legally bound for her support. She has a few households good worth about $25.00. The only other property possessed by Claimant is one horse that is of no value in money, and She has a house & Lot in the town of Long Island Kansas; but the house & Lot is now mortgaged for $100.00 that Claimant has not disposed of any property - real or personal or mixed since Sept 18 1899 ______-that there is a personal indebtedness of $200.00 against Henry Cozad (Deceased) & Mary Cozad his wife. That the facts stated are given from personal knowledge and observation and intimate acquaintance. That said soldier was my brother and I know he was not married previous to his marriage to claimant. That I was present at the wedding of the soldier and claimant in Mercer County Pennsylvania on the ninth day of May 1848 and was a witness to the ceremony of the marriage. That I was well acquainted with both claimant and soldier from said marriage till he died on September 18 1899 and personally know that soldier and claimant lived together as husband and wife from the date of said marriage to the date of his death also that claimant has not remarried since the death of soldier but is still his widow. D. 1900, personally appeared before me a Notary Public in and for the aforesaid County, duly authorized to administer oaths Mary Cozad aged 73 years resident of Long Island County of Phillips State of Kansas well known to me to be reputable and entitled to credit, and who being duly sworn, declared in relation to aforesaid case as follows: I am claimant in above entitled claim and the widow of said soldier that I am not able to furnish a verified copy of a public or church record of my marriage to said soldier because there is no such record and further I am unable to furnish the affidavit of the person who performed the ceremony of marriage because the minister who married me to said soldier is dead and I respectfully request that the affidavits of S M Cozad and Lucinda Montgomery who were present at the marriage of myself and soldier and which are now on file in my claim for accrued pension on Cert No 842823 be accepted in lieu of evidence called for in letter of date Jan 11 1900. Further I desire to say that I have no real estate no stocks bonds or investments of any kind and no income of any kind since Nov 4 1899 and that no one has been legally bound for my support since the death of said soldier. The house and lot I reside on in Long Island Kans is the property of my ______ estate and is worth about 150.00 and is mortgaged for 100.00 my only interest in same is my dower interest and the only personal property I have are household goods to amount $25.00 or $30.00 and one horse that is quite old and of no money value. That said claimant has no money no stocks nor bonds nor investments of any kind and no income or means of support nor is any person legally bound for her support.
