A court of record within and for the County and State aforesaid, Freeborn Garrison, aged 42 years, a resident of _______, County of Pawnee, State of Kansas, who being duly sworn according to law, declares that he is the identical Freeborn Garrison who entered service under the name of Freeborn Garrison on or about the 15 day of Aug 1861 as Private in company B of the 26 regiment of Ill. Inf. Commanded by James B Davis and was honorably discharged in field Georgia on or about the 28 day of Aug, 1864, by reason of Expiration of term of service, that his personal description is as follows: Age 43 years; height, 5 feet 9 inches; complexion, dark; hair, black; eyes, gray. That while a member of the organization aforesaid, in the service and in the line of his duty at Camp Clear Creek in the State of Miss., on or about the 25 day of Sept 1862, he contracted rheumatism affecting his heart, at Snake Creek Gap, Ga. May 1864 the concussion of a passing cannon ball affected his left side. That he was treated in hospitals as follows: by Regimental Surgeon. That he has not been employed in the military or naval service otherwise than as stated above. That since the 28 day of Aug, A.D. 1864, he has not been employed in the military or naval service of the United States. That since leaving the service this applicant he has resided in the City of Lanark of Carroll Co., Ill & Pawnee, in the State of Kansas and his occupation has been that of a farmer and gunsmith. That prior to his entry into the service above named he was a man of good, sound, physical health, being when enrolled a farmer. That he is now Partly disabled from obtaining his subsistence by manual labor by reason of his injuries above described, received in the services of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States. He hereby appoints, with full power of substitution and revocation, GEORGE E. LEMON, of Washington, D.C. his true and lawful Attorney, to prosecute his claim. That he has never received nor applied for a pension. That his Post-office Address is Lyons county of Rice, State of Kansas. Freeborn Garrison He re-filed in 24th day of Oct, A.D. one thousand eight hundred and 87 – he filled in residence as Lyons on the 6 line, his age was 47. Everything else stayed the same. Act of February 6, 1907 Declaration For Pension State of Kansas County of Pawnee ss: On this 10 day of November, A.D. one thousand nine hundred and Ten, personally appeared before me, a Notary Public within and for the County and State aforesaid, a Freeborn Garrison who, being duly sworn according to law, declares that he is 70 years of age, and a resident of Burdett county of Pawnee, State of Kansas; and that he is the identical person who was enrolled at Freeport, Ill under the name of Freeborn garrison as a Private in Comp. B 26 Reg. Illinois Infantry in the service of the United States, in the Civil war, and was honorably discharged at In The Field Georgy, on the 28 day of Aug, 1864. That his personal description at enlistment was as follows: Height. 5 feet 9 inches; complexion, dark; color of eyes, gray; color of hair, Dark; that his occupation was Farmer; that he was born 24 October, 1840, at Delaware Ohio Discharge Says Pennsylvania by Mistake. That his several places of residence since leaving the service have been as follows: Lanark Ill went to this address Feb 1878. That he is a pensioner. That he has heretofore applied for pension 441846. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provision of the Act of February 6, 1907.
