Know ye, That JOHN McLAIN a Private of Captain North's Company (E) 24th
Regiment of Kentucky Volunteer Infantry was mustered... on the ____ day
of _____ One Thousand Eight Hundred and Sixty_____ to serve three years
and is hereby discharged from service by reason of Surgeons Certificate
of disability.

He volunteered at ______ County of Bath State of Kentucky, is 5 feet 10
inches high, Fair complexion, Blue eyes, Grey hair, and by occupation when
enlisted a Wheelwright. This Certificate of Discharge is given said JOHN
McLAIN the 1st day of January, 1863 at Louisville County of Jefferson State
of Kentucky he being actually discharged from service at this date and
place.

JOHN McLAIN, E 24 KY OATH OF IDENTITY. State of Kentucky, County of Montgomery
} SS Personally appeared before the undersigned a Justice of the Peace
for the State and County aforesaid the within described JOHN McLAIN to
me personally known........
