This certificate was issued by the state registrar on 29 August 1974.  On 20 
August 1974, Edgar Lee Curtis, Sr., 8002 Zelma Fields Avenue, Louisville, 
Kentucky, signed the certificate, declaring on oath that the information 
regarding his birth and his parents was accurate.  Supporting evidence [not 
attached] identified as (i) Affidavit of Mrs. Arthur Case -- Age 77 - Aunt, 
made 19 August 1974; (ii) Affidavit of Frank Warren -- Age 75 -- Non-Relative, 
made 19 August 1974; and (iii) #225359 -- Commonwealth Life Insurance Company, 
bearing an original date of 24 December 1938.  

Additional Comments:
Edgar Lee Curtis was a twin of Emmitt Allen Curtis, and also had siblings 
Jewell Christine Curtis and Raymond Cecil Curtis.  Edgar married Jewell Marie 
Oakes on 12 October 1935 at Lexington Avenue Baptist Church, Danville, Boyle 
County, Kentucky.  Edgar passed away on 19 January 2001, in the Norton Suburban 
Hospital, Louisville, Jefferson County, Kentucky, and is buried at the 
Resthaven Memorial Park, Louisville, Jefferson County, Kentucky.
