Bradley aged ____ 60 years who been duly sworn according to law declares that he is the identical George W Bradley who was ENROLLED as a PVT on about 30th day July 1863 in Company E 10th regiment of KY Ca commanded by Capt L Clark _________________ and was honorably DISCHARGED _____________on the ______________________day of __________________18; that Personal description is as follows: age __60______ years; height ____6____feet___1__inches; complexion__dark____hair___dark __eyes__gray_____That while a member of the organization aforesaid in service and in the line of duty at ___Covington______in the state of ___Kentucky_____________ on or about the __first______day of _Sept____ 1863 he contracted a of the lungs and cough has always existed ever since it first made its appearance at the time and place above mentioned said affiant states that he believes the disease of the lungs was the result of exposure that he was subjected to dust and it was the dryest and dustyest that he most ever saw. That he was only in the service about two months and his heart beat got so fast that he was unfit for duty and Colonel Tives sent him home. ? he was never treated ? ? while in the service. That he has not been employed in the military or naval service otherwise then he stated above. That he has not been in the military or naval service of the United States since 22nd day of Sept 1863.
