OBITUARY
     Mr. Forrest H. "Jack" WALLACE, age 78, of Guston, Ky in Meade County, died 
Thursday, January 1, 2004 at Breckinridge Memorial Hospital, Hardinsburg.  He was 
born October 19, 1925, a son of the late Albert D. and Belle STITH WALLACE SCOTT.
He was a member of the Shawnee Masonic Lodge in Louisville, a Kentucky Colonel, a 
Navy veteran of World War II.  A retired Inspector Foreman of Engineers from Civil 
Service, and a member of Patteson Memorial Presbyterian Church in Guston.
    Mr. WALLACE is survived by:
his wife, Annette NORTON WALLACE OF Guston, Ky
two daughters
two sons
one stepson
one sister
8 grandchildren; 3 great grandchildren
and several  nieces and nephews
     Funeral services were held Sunday, January 4, 2004 from the chapel of  the 
Hager Funeral Home in Brandenburg.

#0139 Freedom Cumberland Presbyterian Church & Cemetery; Directions:  Turn North 
immediately west of "Dead Man's Curve" from Kentucky Highway 60.  The church lies 
east of the road, and is blocked from view by a low ridge.
