C.P. BRADBURY, JUDGE The affiant, Bertie Lee Owen, states that she is the same person as Louise Grant mentioned in birth certificate issued by the Commonwealth of Kentucky, and which birth certificate is File No. 1093 and Registration District No. 5333 and which shows that said Louise Grant was born in Bullitt County, Kentucky, on the 2nd day of Jan., 1912, and her father was Wm. O. Grant and mother was Minnie Jane Hays (maiden name) and name of physician was Dr. G.W. Kirk. Affiant states that at time of her birth she was named Louise Grant and her birth certificate as made out by Dr. G.W. Kirk so set out said name but that a few days after said certificate had been made out and filed, her parents decided to call her "Bertie Lee" Grant and that from that time she has been known as and used the name of Bertie Lee instead of Louise and she makes this affidavit for the purpose of having recorded the correct name by which she is known.
