It may have been subsequently filed with it. Originally it may have been part of the set of the other affidavits signed on the same date.] At the time of my being discharged I was a resident of Bullitt Co. Ky. (P.O. Pitts Point, Bullitt Co., Ky.) Since 1864 have lived near Cleveland. My Post Office has been and still is (Cleveland Le Sueur Co. Minn.) Have always been a farmer. The last of August 1863, near Chicamaugu, I contracted disease of heart and lungs, from which I have never recovered. I was with the Regiment two weeks or more before I was sent back to the hospital at Stevenson, Allabama. I never knew the Dr's name who treated me there. After a few days I was transferred to Nashville, to Hospital No. 9. Here I stayed five weeks under medical treatment, but do not know the names of the Dr's who treated me there. There I was transferred to Louisville Ky. to Hospital No. 18. Here Dr. Bell treated me, also gave me a certificate for discharge. After discharge Nov. 11th 1863, I was treated by our family physician Dr Harrold, Pitts Point, Bullitt Co. Ky, till April 1864 when we moved to Minnesota. (Dr Harrold has since died) The Regimental Surgeon that doctored me till I left the Regiment was Dr. Butler (now dead). Only two comrades knew anything about me after my being sent to hospital and the attending circumstances, one of them is dead, the other I know nothing about. None of the officers of the company know any thing about me since discharge. For two years after I was discharged I was unable to do any thing. After comming to Minnesota my health improved a little. For a few years I took Dr. D. Jahnes medicine and did not employ a resident phisian. In 1872 Dr. D. B. Collins (P.O. St. Peter Nicollet Co Minnesota) treated for lung and heart troubles, has doctored me off and on ever since, some years more and some less. For the last six years Ive been unfit for manual labor fully one half the year each year. In the summer of 1877 I was not able to do any thing of account all summer. 1883 the first of August I was taken down and was unable to do any hard work till Spring 1886. From Sept first till Spring I was not in a condition to do any hard work. Have pain between shoulders and in left side with shortness of breath.
