1. I doublechecked my work. The name on the death certificate is "Charlie 
Flack". The sex is "Female", and the occupation is "School girl".

2. The race is actually stated as "Colored" rather than "Black".

3. The exact age is 16 years, 7 months, 24 days.

4. The name of the father is stated as "Jno Flack". So, that's what I typed 
above. It's likely an abbreviation for "John Flack".

5. The informants name is either "J. M. Flack" or else it is "John Flack" and 
the first name is written very poorly. I favor the first.

6. The doctor's statement: "I hereby certify, that I attended deceased from Mch 
25, 1914 to Dec 10, 1914, that I last saw her alive on Nov. 25, 1914, and that 
death occurred on the date stated above at 4:15 p.m."
