3. Doctor's statement: "I hereby certify, that I attended deceased from 12-30, 
1918, to 1-2, 1920, that I last saw him alive on 1-2, 1920, and that death 
occurred on the date stated above at 11 p.m."

Additional Comments:
1. The doctors signature was mostly readable, but strange. It was as if it was 
the signature of a foreign born person.

2. There was an error on the original death certificate where the place of 
burial is written. Instead of filling out a new document, they scratched 
through the error and then tried to write the cemetery name in the corner of 
the text window. I can't read it.
