Bureau of pensions Washington, D.C. -------------------------------------------------------------------------------- Feb,1888------------------------------------------1888 Delacration for the Increase of Invalid Pension State of Kentucky County of Crittenden On this 6th day of February, A.D. one thousand eight hundred and eighty eight, personally appeared before me a County Court Clerk, the same being within and for the county and State aforesaid, Robert H. Ledbetter aged 51 years, a resident of Hurricane County of Crittenden State of Ky. who, being duly sworn according to law, declares that he is a pensioner of the United States duly enrolled at the Chicago Pension Agency, at the rate of $8.00 per month, by reason of disability incurred in the military service of the United States while (1) in Co. A 29 Reg. Ill. Vol. Inf., that his present physical condition is such that he believes himself entitled to receivee an increased pension; and that his present Pension Certificate is No 206,633 and for disability called disease of the Lungs He further declares that he is disabled in the following manner, to-wit: (2) That he ask increase of pension for disease of Lungs as they are now much worse than when last examined and ask *** for Chronic Diahera contacted from ** in the line of his duty at Vicksburn Miss, July 1862, (more unreadable) about day of 1865 he was wounded in the right side by the explosion of a shell. That as a result of the above he has neverous disablities and disease of the heart & Kidneys, that he asks for pension for all of these and all of their effects He claims pension for all effects of the above, and believes he should have $________ a month pension for the future; that he appoints A.W. McCormick & sons of Washington D.C. his true and lawful attorneys to prosecute his claim; that his residence is as above, and his post- office address is Hurricane, Crittenden Co., Ky. Ledbetter, the claimant, sign his name (or make his mark) to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and the acquaintance with him, that he is the identical person he represents himself to be; and that they have no interest in the prosecution of this claim.
