State of Kentucky, County of Hart, ss: On this 23 day of May, A. D. one thousand nine hundred and twelve, personally appeared before me a county court clerk within and for the county and State aforesaid, Washington T. Cook, who, being duly sworn according to law, declares that he is 75 years of age, and a resident of Monroe, county of Hart, State of Kentucky; and that he is the identical person who was ENROLLED at Campbellsville, Kentcky, under the name of Washington T. Cook on the 4 day of Nov, 1861, as a private, in Co. A, 13 Regiment, Ky. Volunteer Infantry, in the service of the United States, in the Civil war, and was HONORABLY DISCHARGED at Louisville, Ky, on the 18 day of January, 1865; that he also served ......; that he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 6 feet 1 inches; complexion fair; color of eyes blur; color of hair Dark; that his occupation was Farming, that he was born in Green County, Ky, 18.., near Center Ky (called Lafeyatt then) on the 16 day of May 1837. That his several places of residence since leaving the service have been as follows: 1st in Green Co. Ky 2 years near Webbs P.O; 2nd in Green Co. Ky near Oceola P.O. 4 years, 3rd near Moody, Texas 2 years, balance of the time in Hart County Ky, near Monroe. That he is a pensioner under certificate No. 401986. That he has ... applied for pension under original No. .... That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provision of the act of May 11, 1912. Cook, a Private, of Company A, 13th Regiment, Ky Infty Volunteers, was enrolled on the 9th day of November, 1861, at Camp Andy Johnson, 3 years and is reported: on Muster Rolls to June 30th/62 sick at home. To Aug 31/62 Captured and paroled; To Dec 31/62 (4 months muster) "paroled prisoner sent to Camp Chace"; Lame report to July 28/63; To April 30/63 present; Lame report on Rolls to April 30/64; To June 30/64 detached on provost Guard duty at Div. HdQrs; Same to Aug 31/64; To Oct 31/64 present; Mustered out with Co. Jan 12/65 at Louisville, Ky. Prisoner of War Records have him captured in Hart Co, Ky July 10/62, paroled in Hart Co, Ky, July 11/62; reported at Camp Chase, Ohio, date not given, when his name appeared on MR dated Oct. 31/62, without remark; again reported at Camp Chase, Ohio, July 27/63. Sent to Reg't Mar 10/63. No evidence of medical treatment or disability on said records. The records of this Office furnish no evidence of disability as alleged. Cook Date of Reply May 4th, 1898 DECLARATION OF A WIDOW FOR ORIGINAL PENSION State of Kentucky County of Hart On this 14th day of June, A.D. one thousand nine hundred and thirteen, personally appeared before me, a county court clerk within and for the county and State aforesaid, Lucy M. Cook, aged 69, and a resident of Monroe, County of Hart, State of Kentucky, who being duly sworn according to law, makes the following declaration in order to obtain pension under the acts of Congress granting pension to the widows of soldiers and sailors who have died by reason of wound or injury received, of disease contracted, in the service of the United States in the line of duty: That she is the widow of Washington T. Cook, who was enrolled under the name of Washington T. Cook at Greensburgh, Ky on the ... day of Sept 1861 as a Private in Co. A, 13 Regiment Ky Volunteer Infantry and was discharged on the 12 day of January 1866, and who died at Monroe, Ky on the 4 day of March 1913 of Hart failure due to Rheumatism incurred in the above-named service. That the said soldier was not in the military or naval service of the United Stat6es except as stated above ....... That the said soldier was born May 16, 1837, at ....... That his personal description at enlistment was as follows: Height ...; complexion...; color of eyes...; color of hair...; permanent marks or scars ...; that his occupation was farming. That she was married under the name of Lucy M. Robinson (lined through) Murray to said soldier at near East Fork Ky on the 19 day of August 1901 by Rev. W. M. Kidd; that there was no legal barrier to the marriage; that she had been previously married; that the soldier had been previously married. (no dates of death or divorce were entered) That she was never divorced from said soldier and that she has not remarried since his death. That the said soldier left the following-named children under 16 years of age at the date of his death, to wit: said soldier left no children under 16 years of age. That she has not heretofore applied for pension. That her post-office address is (street and number) Monroe, RFD, City or town of Monroe, County of Hart, State of Kentucky.
