Report on search of pension & bounty land warrant application files Act of May 11, 1912 Declaration for Pension The Pension Certificate should not be forwarded with the application State of: Kentucky County of: Hart On this 23 day of May, AD 1912, personally appeared before me, a county court clerk within and for the county and State aforesaid, Washington T. Cook, who, being duly sworn according to law, declares that he is 75 years of age and a resident of Monroe, County of Hart, State of Kentucky and that he is the identical person who was enrolled at Campbellsville, Kentucky, under the name of Washington T. Cook on the 4 day of Nov 1861, as a private in Co. A, 13 Regiment KY Volunteer Infantry in the service of the United States Civil War and was honorably discharged at Louisville, KY on the 18 day of January 1865. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height: 6 feet 1 inches; complexion: fair; color of eyes: blue; color of hair: dark; that his occupation was farming that he was born in Green County KY at near Center KY (called Lafeyatt then) on the 16 ! day of May 1837. That his several places of residence since leaving the service have been as follows: 1st in Green Co., KY 2 years near Webbs PO; 2nd in Green Co. KY near Osceola PO 4 years; 3rd near Moody, Texas 2 years; ballance of the time in Hart County KY near Monroe. That he is a pensioner under Certificate No. 401986. That his post office address is Monroe, County of Hart, State of Kentucky Attest(1): Joe E. Lann? (illegible on copy); (2) T. E. Conyers Signed: Washington T. Cook, a private of Company A, 13th Regiment, Ky Inf'ty Volunteers, was enrolled on the 9th day of November 1861, at Camp Andy Johnson, 37 years and is reported on Muster Rolls to June 30th, 1862 ... sick at home to Aug 31, 1862. Captured & paroled; to Dec 31st, 1862 (4 months muster) "Paroled prisoner sent to Camp Chace"; Lame report to Feb'y 28, 1863; to April 30, 1963 present ... Lame report on rolls to April 30, 1864 to June 30th 1864; Detached, on provost Guard duty at Div. HdQrs, same to Aug 31, 1864 ... to Oct. 31, 1864 present; mustered out with Co. Jan. 12, 1865 at Louisville, Ky. Prisoner of War records show him captured in Hart Co., KY July 10, 1862, paroled in Hart Co., KY July 11, 1862; reported at Camp Chase, Ohio, date not given, where his name appeared on MR dated Oct. 31, 1862 without remark, again reported at Camp Chase, Ohio Jan'y 27, 1863. Sent to Reg't Mar. 10 1863. No evidence of medical treatment or disability on said records. The records of this office furnish no evidence of disability as alleged. (Co. A. Cook Date of reply: 5/4/1898 *** Declaration of a Widow for Original Pension State of Kentucky County of Hart On this 14 day of June AD 1913, personally appeared before me, a county court clerk within and for the county and State aforesaid, Lucy M. Cook, aged 69 years, a resident of Monroe, County of Hart, State of Kentucky, who being duly sworn according to law, makes the following declaration in order to obtain pension under the acts of Congress granting pension to the widows of soldiers and sailors who have died by reason of wound or injury received, or disease contracted in the service of the United States and in the line of duty: That she is the widow of Washington T. Cook, who was enrolled under the name of Washington T. Cook in Greensburgh, KY on the ... day of Sept 1861 as a private in Co. A, 13 Regiment, KY Volunteer Infantry and was discharged on the 12 day of January 1865 and who died at Monroe, KY on the 4 day of March 1913 because of hart failure due to Rheumatism incurred n the above named service. That the said soldier was not in the military or naval service of the United States except as stated above.... That the said soldier was born May 16, 1837 ... that his occupation was farming. That she was married under the name of Lucy M. Robinson (lined through) Murray to said soldier at near East fork Ky on the 19 day of August 1901 by Rev. W. M. Kidd; that there was no legal barrier to the marriage; that she had been previously married, that the soldier had been previously married. That she was never divorced from said soldier and that she has not remarried since his death. That said soldier left no children under 16 years of age. That she has not heretofore applied for pension. That her post office address is (street and number) Monroe, RFD, City or town of Monroe, County of Hart, State of Kentucky. Attest: (1) T. A.
