A Declaration for Original Invalid Pension State of Kentucky County of Grayson On this 12 day of January A.D. one thousand eight hundred and eighty personally appeared before me W.C. Wheel__, Dep. County Court Clerk; the same being a Court of Record of the County and State aforesaid William A. Edwards a resident of County of Hart, State of Ky. who being by me duly sworn according to law, on his solemn oath, deposes as follows, to wit: I am the identical William A. Edwards who was enrolled on the 1 day of Jany 1862 to Company I of the 27 Reg’t of Ky by Captain Frank and I was honorably discharged at Louisville Ky on the 29 day of Feby 1862 and my age is 37 years. While in the service aforesaid, and in the line of my duty I received the following disability to wit: In Georgia I took fever which settled in spine. From concussion of exploding shell I was made partially deaf. Treated in hospital no_ Louisville Ky reg. hospital, and hospitals at Knoxville Tenn & Atlanta Ga. was treated by reg. surgeon Claim pension on disease of spine & partial deafness. I have never been in the Military or Naval Service of the United States otherwise than set forth above Since leaving the Service, I have resided in Ky, and my occupation has been farmer before my entry into the Service aforesaid I was of good sound physical health, being at enrollment a farmer and I am now partially disabled from obtaining my subsistence by manual labor by reason of my disabilities above stated, received in the service of the United States, and I make this Declaration for the purpose of being placed on the Invalid Pension Roll of the United States. I hereby appoint and empower, with full power of substitution, Nathan W. Fitzgerald, of Washington City, D.C. my true and lawful Attorney to prosecute my claim. I 27 Ky.Vol. Personally came before me, a notary public in and for the County and State aforesaid, James T. Alvey of Wax Grayson Co., Ky. person of lawful age, who, being duly sworn, declares in relation to the aforesaid claim, as follows My acquaintance with said Wm. A. Edwards dates back to about 1870. At that time he was suffering from and complaining of disease of heart and lungs. He complained of fainting or smothering spells, of difficulty in catching his breath of palpitation and pains in his chest. He coughed often and cleared his throat and spit up much philam and some blood. I have seen him often since my first acquaintance with him and have lived in his immediate neighborhood. He has been taking medicine all the time I have known him. I have often seen him sick in bed and seen him spit blood and have seen his wife fix sand by his bed for him to spit on. I have also seen him so weak he could not turn himself in bed without help. He has not been able to work since I have known him, but is totally disabled for manual labor on account of disease of lungs and heart. Edwards personally comes the claimant, who being first sworn on oath, says: He has wrote to the surgeon of the regiment herewith sends his reply setting forth his reason for not making an affidavit in Claimants Case. He says he cannot furnish medical evidence prior to his enlistment from the fact that he never had any medical attendance prior to enlistment. He says he has had no medical attention since his discharge from the fact that he was satisfied that no physician could restore the fragments of bone that came out of his hip from the result of the fever falling in his spine and hip. Dr. Hest___ his regimental surgeon having told him that he was incurable and he also says he was very poor and had nothing to employ a physician with, and also thinking it unnecessary he did not do so. Edwards Co “I” 27 Ky. Inft. personally comes the claimant, who being first sworn on oath, says: That on or about August the ___ 1864 during the siege or battle of Atlanta Georgia the regiment to which claimant belonged was preparing to charge the enemy. Claimant was in line and the commander finding tho fire too strong ordered the regiment to lie down and as they did so a shell burst near Claimants left side burning his hair and whiskers and prostrating him insensible, and when he came to his mind. in about two hours (his told him about the length of the time) he was much prostrated and the ensuing night the blood run out of his ears, and continued to bleed slightly for about a month causing deafness which has continued to the present time. (Seal) (Official Signature) Stamped U.S.Pension Office Nov 9. 1883 DECLARATION FOR WIDOW’S PENSION ACT OF APRIL 19, 1908 State of Kentucky, County of Hart, 55 On this 23 day of June A.D. one thousand nine hundred and nine personally appeared before me, a notary public within and for the County and State aforesaid Mrs. Nancy J. Edwards aged 66 years, a resident of Dog Creek County of Hart State of Kentucky who, being duly sworn according to law, makes the following declaration in order to obtain pension under the provisions of the ACT OF CONGRESS APPROVED APRIL 10, 1908. That she is the widow of William A. Edwards who was enrolled under the name of William A. Edwards at Grayson Springs on the first day of January 1862 as a private in Company I 27 regiment Kentucky Infantry Volunteers and honorable discharged March the 29 1865 having served ninety days or more during the late war of the rebellion. That the soldier was 3 years and three months in the MILITARY OR NAVAL SERVICE of the United States except as stated above he was appointed 3rd corporal in company I in the 27 regiment of Kentucky Infantry Volunteers on the first day of February 1863. That she married under the name of Nancy J. Herod to said soldier at Brownsville Kentucky on the 16 day of December 1862 by Joseph Minor, that there was no legal ____ that she had not been previously married; that the soldier had not been previously married. That the said soldier died June 17, 1904, at Dog Creek Kentucky that she was not divorced from him; that she has not remarried since his death. That the NAMES and DATES OF BIRTH of all the children of the soldier, now living and under SIXTEEN YEARS OF AGE are as follows no children under sixteen years old That she has not heretofore applied for pension That she hereby appoints, with full power of substitution and revocation,- U.S.G.Vanfleet of Munfordville Ky her true and lawful attorney to prosecute this claim, and directs that the sum of Ten Dollars be paid for his service.
