Butler, Civil War, Russell & Wayne Counties ------------------------------------------------------------ Declaration For Original Invalid Pension State of Kentucky Russell County On this 18 day of December,AD one thousand eight hundred and ninty one personally appeared before me, a dept. clk of the Russell County Court, a court of record within and for the County and State aforesaid, Daniel J. Butler aged 51 years, a resident of the town of Kendall, county of Russell, State of Kentucky, who being duly sworn according to law,declares that he is the identical Daniel J. Butler who was ENROLLED on the early part, 1861 in Company H of the 12th Regiment of Ky Infty commanded by William A. Hoskins and was ???? at Nashville Tenn. about the first day of June 1862;that his personal description is as follows: Age 51 years;height 6 feet; complection, fair; Hair, dark; eyes Dark That while a member of the organization aforesaid, in the service and in the line of duty at Camp Dick Roberson, in the State of Ky on or about the first day of September 1861, he contracted Lung trouble ??? of a spell of measles that he was treated ....He was treated in hospitals as follows:at Somerset Ky about Sept. 1861 That he has not been employed in the military or naval service otherwise than as stated above That since leaving service the applicant has resided in the Counties of Wayne, Clinton, and Russell in the State of Ky., and his occupation has been that of a farmer That prior to his service above named he was a good,sound, physical health,being when enrolled a farmer That he is now farmer disabled from obtaining his substience by manual labor by reason of his injuries, above described, received in the Service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States He hereby appoints, with full power of substitution, J F CARNES of Kendall Ky his true and lawful attorney to prosecute his claim.
