Lincoln County. Minnie E. Phillips of said State and County, desiring to avail herself of the pension allowed to Indigent Widows of Soldiers, General Assembly, approved March 18, 1914, hereby submits her proofs, and after being duly sworn true answers to make to the following questions, deposes and answers as follows: 1. What is your full name and where do you reside? Minnie E. Phillips, reside at Stanford, R.F.D.#3, Lincoln County, Kentucky. 2. How long and since when have you been a resident of this State? All my life, and will be 59 yrs old Nov. 13th 1929. 3. When and where were you born and what was your maiden name? Born at Parksville, Boyle County, Ky. on Nov. 13th 1870, and my maiden name was Minnie E. Rousey. 4. When and where was your husband born-state his full name, and where were you and he married, and who performed the marriage ceremony. He was born Oct. 18th 1829 in the State of Georgia, but the exact location, I do not know, and his full name was John Thomas Phillips; we were married in Boyle County, Ky, on Nov. ? 1887 by Rev. Harry Witon. 5. When and where and in what Company and Regiment did your husband enlist or serve during the war between the States? He enlisted in the State of Georgia, but don't know exact time, but served in the 43rd Battalion Virginia Cavalry, Co. G. under John S. Moseby. 6. How long did your husband serve in said Company and Regiment? 3 years or thereabout. 7. When and where did your husband's Company and Regiment surrender? in 1865 at Chi______. 8. Was your husband present at the time and place when his Company and Regiment surrendered? yes. 9. If not with his command at surrender, state clearly and specifically where he was, when he left command, for what cause and by what authority?______ 10. When and where did your husband die? died September 14th 1917 Lincoln County, Ky. 11. At the time of your husband's death, were you living with him as his lawful wife? yes. 12. Have you married since the death of your soldier husband? No. 13. What property, real or personal, or income do you have or possess, and its gross value? Have no property of any kind, and the only income I have is by days work, and living with my children. 14. Have you a family? If so, who compose such family? I am not keeping house and am living with my children, and I have ten living children, all grown, said children being by my deceased husband John Thomas Phillips. 15. Name some friend, giving his name and postoffice address, who will be willing to have us write to him about your case if necessary Kelly J. Francis, Stanford, Kentucky.
