1. The name of the physician appears to be Menton Faulkner. However it's 
possible the first name is misspelled. The doctor signed the certificate on 
Jan. 11, 1912.

2. No hospital names are mentioned.

3. The location of the Flack Graveyard is not stated. I assume it is near Mt. 
Sterling, Kentucky. That is the address for the undertaker.

4. I abbreviated the name "George" as "Geo." because it was abbreviated on the 
death certificate.

5. The place of residence is not stated on the certificate.
