Length of stay: 6 days (at place of death) 2. For the question, "Married, Never Married, Widowed, Divorced" the certificate states "Sept". I assume that that means he was separated from his wife. In truth, his second wife, Ruby Lee Flack, died of a gunshot wound. 3. The exact time of day for the death is unreadable because the doctor stamped the certificate over top the time space and blotted out that information. 4. The doctor's names (two were there) are illegible and the names stated above are my best guesses. 5. The funeral home name is illegible, but seems to be saying "Shannon Funeral Service". Additional Comments: 1. His mother's name is spelled "Happer" on the death certificate. However, her name was actually Bernice Harper. 2. The place of birth for the parents is not stated on the death certificate. 3. The father, John Flack, is the son of James J. Flack and Margaret Jane Chisholm, both of Henry County, Kentucky.
