All rights reserved. http://usgwarchives.net/copyright.htm http://usgwarchives.net/la/lafiles.htm ********************************************** State of Louisiana Parish of Catahoula Personally appeared before me the undersigned authority Jefferson Ezell who being duly sworn says that he commenced the practice of Medicine and Surgery at Winnfield in Winn Parish Louisiana in the year 1856, and remained in the vicinity for about six weeks after which time he removed to the Parish of Catahoula and commenced the practice of Medicine in said Parish in July 1856, and that he continued to so practice until Sept 1872, at which time affiant went to Louisville Kentucky and attended the University of Louisville and there received the degree of Doctor of Medicine in March 1873. Affiant before commencing the practice first attended Lectures of the Medical School of New Orleans (Known as the old School) in the winters of 1855, 56, & 1857 making two courses of lectures at New Orleans. Was Born in Pike County State of Mississippi Oct 2d 1831 and is 51 years of age. And that he never practiced Medicine outside of the State of Louisiana and that his Post office address is Jena Catahoula Parish. Sworn to & signed before me Jefferson Ezell Dec 14th 1882 (Seal) John Dosher, Clerk of the 7th District Court Parish of Catahoula 7th District Court State of Louisiana Clerks Office I, hereby certify the within affidavit has been duly recorded in a Book kept By me for the purpose of Recording the affidavits of Physicians, and that the original Affidavit has been duly sent to the office of the Board of health at New Orleans in Conformity to Act No 31 of the General Assembly of Louisiana Relative to the practice of Medicine in the State of Louisiana and that said affidavit was Recorded Dec 14th 1882 by me in said Book for the Parish of Catahoula.
