I, Fred Moscary, aged 45 years, occupation farmer, do 
declare on oath that my personal description is: Color -
white, complexion - brown, height - 5 feet 4 inches,
weight - 146 pounds, color of hair - dark brown, color of
eyes - blue, other visible distinctive marks - none, was
born in Tolscno, Hungary, on the 24th day
of May, anno Domini 1873; I now reside at 152 W. 118th St.,
Chicago, Illinois.
I emigrated to the United States of America from Antwerp, 
Belgium, on the vessel Zealand; my last foreign
residence was Tolscno, Hungary.  I am not married; the name
of my wife is ----; she was born at ---- and now resides at
---------.

It is my bona fide intention to renounce forever all allegiance
and fidelity to any foreign prince, potentate, state, or sov-
ereignty, and particularly to Charles, Emperor of 
Austria and Apostolic Kine of Hungary, of whom I am now a
subject; I arrived at the port of New York, in the state of
New York, on or about the 10th day of January, anno Domini 1903;
I am not an anarchist; I am not a polygamist nor a believer
in the practice of polygamy; and it is my intention in good
faith to become a citizen of the United States of America
and to permanently reside therein; SO HELP ME GOD.
