DECLARATION OF INTENTION
(Invalid for all purposes seven years after the date hereof)

Summit County }        of Summit County


I, George Tedio, aged 33 years, occupation carpenter, do 
declare on oath that my personal description is: Color -
white, complexion - dark, height - 5 feet 7 inches,
weight - 145 pounds, color of hair -  black, color of
eyes - blue, other visible distinctive marks - none, was
born in Nemit Ker, Hungary, on the 12th day
of April, anno Domini 1886; I now reside at 159 Anton Ct.,
Akron, Ohio.

I emigrated to the United States of America from Fiume, 
Hungary, on the vessel Carpathia; my last foreign
residence was Nemit Ker, Hungary.  I am not married; the name
of my wife is blank; she was born at blank and now resides at blank.

It is my bona fide intention to renounce forever all allegiance
and fidelity to any foreign prince, potentate, state, or sov-
ereignty, and particularly to Charles, Emperor of 
Austria and Apostolic Kine of Hungary, of whom I am now a
subject; I arrived at the port of New York, in the state of
New York, on or about the 18th day of April, anno Domini 1911;
I am not an anarchist; I am not a polygamist nor a believer
in the practice of polygamy; and it is my intention in good
faith to become a citizen of the United States of America
and to permanently reside therein; SO HELP ME GOD.
