Be it Remembered, That on this day to-wit: the Twelfth of June 1893, 
... before me S. R. Olliphant, M.D. ... in and for the Parish and City 
of New Orleans, 

personally appeared, 
A. Lamothe, native of La. residing in this city who hereby declares, 
that on the twelfth of June 1887 (June 12, 1887) at No. 253 Bourbon 
St. was born a male child, named 

			John Sidney Baudier (white) 

lawful issue of Jno. A. Baudier a native of N. O., La. aged 38 years, 
occupation Clerk and Louise A. Baudier a native of N. O., La. aged 28 
years. 

Thus done at New Orleans in the presence of the aforesaid A. Lamothe.
