Be it Remembered, That on this day to=wit: the Twelfth of June in 
the year of our Lord One Thousand Eight Hundred and Ninety Three 
[12 June 1893] ... before me, S. R. Olliphant, M. D., ... in and 
for the Parish and City of New Orleans, 

personally appeared 
A. Lamothe, native of La. residing in this city who hereby declares, 
that on the Twentieth of May 1891 (May 20th 1891) at No. 253 Bourbon 
St. was born a female child, named 

			Mary Rita Baudier (White) 

Lawful issue of Jno. A. Baudier a native of N. O., La. aged 42 years, 
occupation Clerk and Louise A. Baudier a native of N. O., La., aged 
32 years, 

Thus done at New Orleans, in the presence of the aforesaid A. Lamothe ...
