Be it Remembered, That on this day to-wit. the Nineteenth of July 
in the year of our Lord One Thousand Eight Hundred and Ninety Four 
and the One Hundred and Nineteenth of the Independence of the United 
States of America before me, S. R. Olliphant, M. D., President Board 
of Health and ex-officio Recorder of Births, Deaths and Marriages, in 
and for the Parish and City of New Orleans, 

personally appeared :
Percy J. Bernard, native of New York City, residing at no. 63 Villere
___ Algiers who hereby declares that on the thirty first day of January 
1894 (31 Jan 1894) at his ___was born a Male child, named 

			Arthur James Bernard (white) 

Lawful issue of Deponent anative of New York City aged 49 years, 
occupation Painter and Mary Smith a native ofIreland, aged 27 years,
 
Thus done at New Orleans, in the presence of the afore said P. J. Bernard 
as also in that of Messrs. ___ Lanauzre and L. B. ___ both of this City 
witnesses, by merequested so to be, who have hereunto set their hands 
together with me, after duereading hereof, the day, month and year first
