Be it Remembered, That on this day to-wit: the Fifteenth of June 
in the year of  our Lord One Thousand Eight Hundred and Ninety-four 
and the One Hundred and Eighteenth of the independence of the United 
States of America, before me, S. R. Olliphant, M. D., President Board 
of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in 
and for the Parish and City of New Orleans, 

personally appeared 
????????, a native of Louisiana residing in Carrollton who hereby 
declares, that on the Twelfth day of April 1888 (April 12, 1888) 
at 73 Washington Street was born a Female child, named 

			Augusta Leontine Boesch (White) 

Lawful issue of George Boesch a native of Louisiana aged 30 years, 
occupation Levee Foreman and Wilhelmina Ahrens a native of Germany 
aged 26 years.

Thus done at New Orleans, in the presence of the aforesaid ???????? as 
also in that of Messrs. P. Henry Lanauze and A. P. Smith both of this 
City witnesses, by me requested so to be, who have hereunto set their 
hands together with me, after due reading hereof, the day, month and 
year first above written.
