Be it Remembered That on this day to-wit the Twentieth of April 
in the year of our Lord One Thousand Eight Hundred and Ninety 
Six One Hundred and 20 of the independence of the United States 
of America, before me, S. R. Olliphant, MD, President Board of 
Health and Ex-Officio Recorder of Births, Deaths, Marriages, in 
and for the Parish and City of New Orleans, 

personally appeared 
ROSARIO CARUBBA, native of Italy residing on Vallette and Evelina 
Streets Algiers, who hereby declares that on the Twentieth of June 
1894 (20 June 1894) at his residence was born a female child, named 

			CONCETTA CARUBBA (white) 

Lawful issue of Deponent a native of Italy aged 25 years, occupation 
Shoemaker and Carmela Palizzi (sp. incorrectly) a native of Province 
of Italy, aged 19 years, 

Thus done at New Orleans, in the presence of the aforesaid R. CARUBBA 
as also in that of Messrs. P. H. Lanauze and Jas. T O Dowd both of 
this city witnesses, by me requested so to be, who have hereunto set 
their hands together with me, after due reading hereof, the day, month 
and year first above written.
